ภาระการรักษาในผู้สูงอายุโรคเบาหวานในชุมชน: ความท้าทายและกลยุทธ์สนับสนุน จากระบบบริการสุขภาพปฐมภูมิ

โรคเบาหวานนับเป็นปัญหาด้านสุขภาพที่สำคัญในกลุ่มผู้สูงอายุ โดยผู้สูงอายุจำนวนมากต้องเผชิญกับ “ภาระจากการรักษา” ซึ่งหมายถึงภาระงานและความรับผิดชอบที่ผู้ป่วยต้องแบกรับในการจัดการตนเองตามแผนการรักษา ได้แก่ การรับประทานยา การปรับเปลี่ยนพฤติกรรมสุขภาพ การติดต่อประสานงานกับหน่วยบริการสุขภาพ โดยเฉพาะผู้สูงอายุในชุมชน ซึ่งมักเผชิญข้อจำกัดด้านการเข้าถึงบริการสุขภาพและขาดความรอบรู้ด้านสุขภาพ ภาระเหล่านี้อาจส่งผลกระทบทั้งการไม่ยึดมั่นต่อแผนการรักษา และคุณภาพชีวิตที่ลดลง
บทความวิชาการนี้เป็นการสังเคราะห์เชิงแนวคิดเกี่ยวกับภาระจากการรักษา มีวัตถุประสงค์เพื่อนำเสนอการวิเคราะห์อย่างครอบคลุมเกี่ยวกับบริบท และองค์ประกอบของภาระจากการรักษาในผู้สูงอายุโรคเบาหวานในชุมชน ประกอบด้วย 1) มิติของภาระจากการรักษา 2) แนวทางการประเมินภาระจากการรักษา 3) ปัจจัยที่ส่งผลต่อภาระจากการรักษา 4) ความท้าทายของระบบสุขภาพปฐมภูมิในการลดภาระจากการรักษาของผู้สูงอายุโรคเบาหวาน และ 5) กลยุทธ์สนับสนุนจากระบบสุขภาพปฐมภูมิเพื่อลดภาระจากการรักษาในผู้สูงอายุ
ระบบสุขภาพปฐมภูมิในฐานะกลไกหลักในการดูแลสุขภาพของผู้สูงอายุในชุมชน ควรออกแบบบริการที่ไม่เพิ่มภาระให้กับผู้สูงอายุ ในขณะเดียวกันช่วยส่งเสริมให้ผู้สูงอายุสามารถดำรงชีวิตประจำวันได้อย่างมีคุณภาพ และสอดคล้องกับบริบทเฉพาะของตนเองอย่างแท้จริง เพื่อยกระดับคุณภาพชีวิตของผู้สูงอายุที่เป็นโรคเบาหวานอย่างยั่งยืนในระยะยาว
