การพัฒนาแนวปฏิบัติการดูแลผู้ป่วยที่ใส่สายระบายทรวงอกเพื่อป้องกันการเลื่อนหลุดและอุดตัน ในผู้ป่วยหลังผ่าตัดหัวใจแบบเปิด: การทดลองใช้

บทคัดย่อ

การเลื่อนหลุดและอุดตันของสายระบายทรวงอก เป็นปัญหาที่พบบ่อยในผู้ป่วยที่ใส่สายระบายทรวงอกหลังผ่าตัดหัวใจแบบเปิด ทำให้เกิดภาวะแทรกซ้อนที่ส่งผลกระทบต่อคุณภาพการพยาบาล งานวิจัยนี้ มีวัตถุประสงค์เพื่อพัฒนาแนวปฏิบัติการดูแลผู้ป่วยที่ใส่สายระบายทรวงอกเพื่อป้องกันการเลื่อนหลุดและอุดตัน โดยใช้กรอบแนวคิดการใช้หลักฐานเชิงประจักษ์ของ ซูคัพ ขั้นตอนดำเนินการวิจัยประกอบด้วย 3 ระยะ ได้แก่ 1) การทบทวนปัญหาในหน่วยงาน 2) การสืบค้นหลักฐานเชิงประจักษ์ และ 3) การนำแนวปฏิบัติไปทดลองใช้ เครื่องมือที่ใช้เก็บข้อมูล คือ แบบบันทึกอุบัติการณ์เลื่อนหลุดและอุดตันของสายระบายทรวงอก และแบบประเมินความพึงพอใจของพยาบาล วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา

ผลการวิจัยพบว่า แนวปฏิบัติการดูแลผู้ป่วยที่ใส่สายระบายทรวงอกเพื่อป้องกันการเลื่อนหลุดและอุดตัน ประกอบด้วย 1) การดูแลสายระบายทรวงอก 2) การดูแลแผลรอบสายระบายทรวงอก 3) การดูแลขวดระบาย และ 4) การประเมินภาวะแทรกซ้อน แนวปฏิบัติผ่านการพิจารณาคุณภาพโดยผู้ทรงคุณวุฒิ 5 คน มีค่าดัชนีความตรงเชิงเนื้อหา เท่ากับ 0.95 ผลของการนำแนวปฏิบัติไปทดลองใช้กับผู้ป่วยที่ใส่สายระบายทรวงอกหลังผ่าตัดหัวใจแบบเปิด ที่รักษาในหอผู้ป่วยหนักศัลยกรรมหัวใจและทรวงอก โรงพยาบาลขอนแก่น จำนวน 2 คน พบว่าหลังผ่าตัด 72 ชั่วโมง ไม่พบอุบัติการณ์เลื่อนหลุดหรืออุดตันของสายระบายทรวงอก และการนำแนวปฏิบัติไปทดลองใช้กับพยาบาลที่ปฏิบัติงานในหอผู้ป่วยเดียวกัน จำนวน 3 คน พบว่า พยาบาลมีความพึงพอใจร้อยละ 94

ผลการวิจัยชี้ให้เห็นถึงความเป็นไปได้ในการนำแนวปฏิบัติไปใช้ และได้มีการปรับลดความซ้ำซ้อนของกิจกรรมการพยาบาล อย่างไรก็ตาม ควรมีการขยายขอบเขตของการศึกษาไปยังหน่วยงานอื่นที่มีบริบทใกล้เคียงกัน ก่อนการนำไปประยุกต์ใช้ในทางคลินิกต่อไป
