บทคัดย่อ

ความไม่สมดุลของความพยายามกับรางวัลที่ได้รับ (ERI) เป็นปัจจัยสำคัญที่มีอิทธิพลต่อการคงอยู่ในงาน อย่างไรก็ตาม ยังไม่มีการทบทวนงานวิจัยที่สรุปความรู้เกี่ยวกับความไม่สมดุลของความพยายามกับรางวัลที่ได้รับของพยาบาล การทบทวนแบบกำหนดขอบเขตในครั้งนี้ มีจุดประสงค์เพื่อศึกษาสถานการณ์ความไม่สมดุลของความพยายามกับรางวัลที่ได้รับ ปัจจัยเหตุและผล และข้อเสนอแนะจากการวิจัยเกี่ยวกับความไม่สมดุลของความพยายามกับรางวัลที่ได้รับในพยาบาล โดยดำเนินการตามกรอบแนวคิดของ อัสกี้ และ โอมอลเลย์ ค้นหางานวิจัยที่ตีพิมพ์ในปี ค.ศ. 2014 ถึง 2024 ผลการสืบค้นพบบทความทั้งหมด 109 เรื่อง หลังจากคัดกรองโดยใช้แบบคัดกรองงานวิจัย และแบบบันทึกผลการสกัดข้อมูลพบว่า มี 21 เรื่องผ่านการคัดกรองตามเกณฑ์ และนำมาวิเคราะห์เชิงเนื้อหาเพื่อสรุปข้อค้นพบสำคัญตามวัตถุประสงค์ของการทบทวนแบบกำหนดขอบเขต

ผลการทบทวน มีดังนี้
1. การรายงานสถานการณ์ความไม่สมดุลของความพยายามกับรางวัลที่ได้รับ มี 2 แบบ ได้แก่ รายงานเป็นจำนวนหรือร้อยละของกลุ่มพยาบาลที่มีคะแนน ERI แตกต่างกัน และรายงานค่าเฉลี่ยระดับ ERI โดยงานวิจัยจำนวน 11 เรื่อง รายงานว่ามีความสมดุล (ERI ≤ 1) และมีงานวิจัย 10 เรื่อง รายงานว่ามีความไม่สมดุล (ERI > 1) ของอัตราส่วนของความพยายามกับรางวัลที่ได้รับในพยาบาล
2. ปัจจัยเหตุของความไม่สมดุลของความพยายามกับรางวัลที่ได้รับในพยาบาล สามารถจำแนกได้เป็น 3 กลุ่ม คือ ปัจจัยด้านองค์กร (เช่น ความขัดแย้งกับหัวหน้างาน การขาดการสนับสนุน การถูกทำร้าย) ปัจจัยด้านลักษณะงาน (เช่น แผนกที่ปฏิบัติงาน การทำงานแบบหมุนเวียนกะ และการทำงานล่วงเวลา) และปัจจัยส่วนบุคคล (เช่น ข้อมูลส่วนบุคคล สุขภาพจิต)
3.  ผลของความไม่สมดุลของความพยายามกับรางวัลที่ได้รับในพยาบาล สามารถจำแนกได้เป็น 2 กลุ่ม คือ ผลต่อสุขภาพของพยาบาล (เช่น ความเครียด ความวิตกกังวล และภาวะหมดไฟ) ผลต่อองค์กร (เช่น คุณภาพการดูแลผู้ป่วยลดลง ความพึงพอใจในงานน้อยลง ความตั้งใจลาออกจากงานเพิ่มขึ้น)
4. ข้อเสนอแนะของผลการวิจัยต่อผู้บริหารองค์กรและผู้บริหารทางการพยาบาล ได้แก่ การดูแลสวัสดิการของพยาบาล จัดอบรมและให้คำปรึกษาเกี่ยวกับวิธีการจัดการกับความเหนื่อยล้า ความเครียด และความขัดแย้ง สร้างสภาพแวดล้อมการทำงานที่ปลอดภัย และส่งเสริมการให้รางวัลแก่พยาบาล

ผลการทบทวนแบบกำหนดขอบเขตนี้ เน้นให้เห็นถึงความไม่สมดุลของความพยายามกับรางวัลที่ได้รับในพยาบาล และให้ข้อมูลเชิงลึกแก่ผู้บริหารโรงพยาบาล เพื่อสามารถนำไปใช้ในการพัฒนากลยุทธ์เพื่อให้เกิดความสมดุล และรักษาพยาบาลให้คงอยู่ในองค์กร
