ผลของโปรแกรมส่งเสริมการเปลี่ยนผ่านสู่บ้านต่อความมั่นใจในการดูแลของมารดา และการกลับมารักษาซ้ำของทารกเกิดก่อนกำหนดระยะท้าย

ทารกเกิดก่อนกำหนดระยะท้ายมีลักษณะคล้ายทารกครบกำหนด ทำให้ได้รับการดูแลและจำหน่ายใกล้เคียงกัน อย่างไรก็ตาม ทารกกลุ่มนี้ยังคงมีความเสี่ยงต่อภาวะแทรกซ้อนทางสุขภาพและอาจต้องกลับมารักษาซ้ำได้ การศึกษาวิจัยแบบกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อเปรียบเทียบความมั่นใจในการดูแลของมารดาและการกลับมารักษาซ้ำของทารกเกิดก่อนกำหนดระยะท้าย ระหว่างกลุ่มทดลองที่ได้รับโปรแกรมส่งเสริมการเปลี่ยนผ่านสู่บ้าน และกลุ่มควบคุมที่ได้รับการพยาบาลปกติ กลุ่มตัวอย่างคือ มารดาและทารกเกิดก่อนกำหนดระยะท้ายจำนวน 54 คู่ คัดเลือกแบบเฉพาะเจาะจง แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 27 คู่เท่ากัน เครื่องมือวิจัย ได้แก่ (1) โปรแกรมส่งเสริมการเปลี่ยนผ่านสู่บ้าน ซึ่งประกอบด้วยการประเมินความพร้อม การเตรียมตัวสำหรับการเปลี่ยนผ่านที่มีเนื้อหาเจาะจงกับทารกเกิดก่อนกำหนดระยะท้าย และการเสริมบทบาทด้วยการติดตามผ่าน LINE OA (2) แบบสอบถามความมั่นใจในการดูแลของมารดา และ (3) แบบบันทึกการมารักษาซ้ำ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติ Mann–Whitney U test, Friedman test และ Wilcoxon signed-rank test
ผลวิจัยพบว่า กลุ่มทดลองมีคะแนนความมั่นใจในการดูแลของมารดาเพิ่มขึ้นจากก่อนเข้าร่วมโปรแกรม (Mdn = 58.00, IQR [48.00, 63.00]) โดยเพิ่มขึ้นหลังจำหน่ายที่สัปดาห์ที่ 1 (Mdn = 63.00, IQR [60.00, 65.00]) และสัปดาห์ที่ 4 (Mdn = 67.00, IQR [64.00, 70.00]) อย่างมีนัยสำคัญทางสถิติ และเมื่อเปรียบเทียบกับกลุ่มควบคุม คะแนนความมั่นใจหลังจำหน่ายทั้งสองช่วงเวลาสูงกว่าอย่างมีนัยสำคัญทางสถิติ (Z = -2.84, p < .05 and Z = -3.52, p < .01 ตามลำดับ) ส่วนอัตราการกลับมารักษาซ้ำภายใน 28 วันหลังเกิดของกลุ่มทดลองและกลุ่มควบคุมคิดเป็นร้อยละ 11.11 และร้อยละ 14.81 ตามลำดับ โดยไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ
การศึกษาครั้งนี้แสดงให้เห็นว่า โปรแกรมส่งเสริมการเปลี่ยนผ่านสู่บ้านมีประสิทธิผลสามารถช่วยส่งเสริมความมั่นใจในการดูแลของมารดาทารกเกิดกำหนดระยะท้าย แม้จะยังไม่สามารถลดอัตราการกลับมารักษาซ้ำได้อย่างชัดเจน ดังนั้น งานวิจัยในอนาคตควรเพิ่มเนื้อหาเกี่ยวกับการดูแลภาวะตัวเหลืองขณะทารกอยู่ที่บ้านซึ่งเป็นสาเหตุสำคัญของการกลับมารักษาซ้ำ เพื่อเพิ่มศักยภาพของโปรแกรมและลดภาระการรักษาพยาบาลในระยะยาว
