ผลของโปรแกรมส่งเสริมการพยาบาลในการจัดการความปวดต่อการปฏิบัติการพยาบาล และความปวดของทารกเกิดก่อนกำหนดที่เข้ารับการตรวจคัดกรอง ภาวะจอประสาทตาผิดปกติ

ภาวะจอประสาทตาผิดปกติในทารกเกิดก่อนกำหนด (ROP) เป็นภาวะที่มีการเจริญผิดปกติของหลอดเลือดจอประสาทตา พบมากในทารกเกิดก่อนกำหนด กระบวนการตรวจคัดกรอง ROP ทำให้ทารกได้รับความปวดมาก การวิจัยกึ่งทดลองนี้เป็นแบบสองกลุ่มวัดผลก่อนหลังการศึกษา โดยมีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการส่งเสริมพยาบาลในการจัดการความปวดของทารกเกิดก่อนกำหนดที่ได้รับการตรวจคัดกรองภาวะจอประสาทตาผิดปกติต่อการปฏิบัติพยาบาล และการตอบสนองต่อความปวดของทารก กลุ่มตัวอย่างเป็นพยาบาลที่ปฏิบัติงานในหอผู้ป่วยหนักทารกแรกเกิด และหอผู้ป่วยวิกฤตทารกแรกเกิด จำนวน 20 คน และทารกเกิดก่อนกำหนดที่ได้รับการตรวจ ROP จำนวน 54 เหตุการณ์ เครื่องมือที่ใช้ในการวิจัยประกอบด้วย 2 ส่วน ได้แก่ (1) เครื่องมือที่ใช้ในการดำเนินการวิจัย คือ โปรแกรมการส่งเสริมพยาบาลในการจัดการความปวดของทารกเกิดก่อนกำหนดที่ได้รับการตรวจคัดกรอง ROP และ (2) เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบสังเกตการปฏิบัติของพยาบาลในการจัดการความปวด และแบบประเมินความปวดของทารก วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติ Wilcoxon signed-rank test, Mann-Whitney U test และ Friedman test
ผลการศึกษาพบว่า หลังได้รับโปรแกรมฯ กลุ่มทดลองมีคะแนนมัธยฐานการปฏิบัติของพยาบาลในการจัดการความปวดของทารกเกิดก่อนกำหนดที่ได้รับการตรวจคัดกรอง ROP สูงกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติ (Mdn = 20.00, IQR = 1.00 vs. Mdn = 15.00, IQR = 2.00) (Z = -2.345, p = .019) และคะแนนมัธยฐานความปวดของทารกเกิดก่อนกำหนดในกลุ่มทดลองหลังตรวจ 1 นาที และ 5 นาที ต่ำกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติ (Z = -4.453, p < .001; Z = -3.767, p < .001)
การสนับสนุนการใช้โปรแกรมการส่งเสริมพยาบาลในการจัดการความปวดของทารกเกิดก่อนกำหนดที่ได้รับการตรวจคัดกรองภาวะจอประสาทตาผิดปกติตามแนวคิด PRECEDE-PROCEED สามารถเพิ่มประสิทธิภาพการปฏิบัติหน้าที่ของพยาบาลได้ดีขึ้น และช่วยลดความปวดของทารกเกิดก่อนกำหนดที่ได้รับหัตถการดังกล่าว
