การพัฒนาโปรแกรมการสอนแนะร่วมกับใช้แอปพลิเคชันไลน์ต่อพฤติกรรมการดูแลตนเอง เพื่อป้องกันภาวะน้ำเกินในผู้ป่วยล้างไตทางช่องท้อง: การวิจัยและพัฒนา

ภาวะน้ำเกินมีความสัมพันธ์กับการเพิ่มขึ้นของอัตราการเจ็บป่วยและการเสียชีวิต ในผู้ป่วยที่ได้รับการล้างไตทางช่องท้อง การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อพัฒนาโปรแกรมการสอนแนะร่วมกับใช้แอปพลิเคชันไลน์ และเปรียบเทียบพฤติกรรมการดูแลตนเองเพื่อป้องกันภาวะน้ำเกินและอุบัติการณ์การเกิดภาวะน้ำเกินในผู้ป่วย ระหว่างกลุ่มทดลองกับกลุ่มควบคุม กลุ่มตัวอย่าง คือ ผู้ป่วยไตเรื้อรังระยะที่ 5 ที่ได้รับการบำบัดทดแทนไตด้วยการล้างไตทางช่องท้อง ณ คลินิกล้างไตทางช่องท้อง โรงพยาบาลม่วงสามสิบ จำนวน 60 คน เลือกตัวอย่างแบบเจาะจง แบ่งออกเป็นกลุ่มทดลอง จำนวน 30 คน ที่ได้รับโปรแกรมการสอนแนะร่วมกับใช้แอปพลิเคชันไลน์ และกลุ่มควบคุม จำนวน 30 คน ที่ได้รับการพยาบาลตามมาตรฐาน เครื่องมือวิจัยประกอบด้วย โปรแกรมการสอนแนะร่วมกับใช้แอปพลิเคชันไลน์ แบบสอบถามข้อมูลส่วนบุคคลและสุขภาพ แบบสอบถามพฤติกรรมการดูแลตนเอง และแบบบันทึกอุบัติการณ์การเกิดภาวะน้ำเกิน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติ chi-square test, risk difference, independent sample t-test และ ANCOVA
ผลการวิจัยพบว่า
1. โปรแกรมการสอนแนะร่วมกับใช้แอปพลิเคชันไลน์ มีค่าดัชนีความตรงเชิงเนื้อหาอยู่ระหว่าง 0.82-1.00 ถือว่าอยู่ในเกณฑ์ที่ยอมรับได้
2. เมื่อควบคุมคะแนนพฤติกรรมการดูแลตนเองเพื่อป้องกันภาวะน้ำเกินก่อนการทดลองแล้วพบว่า คะแนนพฤติกรรมการดูแลตนเองเพื่อป้องกันภาวะน้ำเกินในผู้ป่วยที่ล้างไตทางช่องท้องภายหลังการทดลองของกลุ่มทดลองสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (F(1, 57) = 342.03, p < .001)
3. อุบัติการณ์การเกิดภาวะน้ำเกินในผู้ป่วยที่ล้างไตทางช่องท้องของกลุ่มทดลอง (ร้อยละ 13.33) ต่ำกว่ากลุ่มควบคุม (ร้อยละ 43.33) อย่างมีนัยสำคัญทางสถิติ (c2 = 6.648, p < .05)
ผลการศึกษาครั้งนี้ให้ข้อเสนอแนะว่า โรงพยาบาลระดับชุมชนสามารถนำโปรแกรมนี้ไปประยุกต์ใช้กับผู้ป่วยที่ล้างไตทางช่องท้อง โดยเฉพาะกิจกรรมตามโปรแกรมร่วมกับใช้แอปพลิเคชันไลน์เพื่อป้องกันภาวะน้ำเกินในผู้ป่วยที่ล้างไตทางช่องท้องได้
