ปัจจัยทำนายภาวะเปราะบางของผู้สูงอายุโรคต่อมลูกหมากโต

ภาวะเปราะบาง เป็นกลุ่มอาการทางคลินิกที่พบได้บ่อยในผู้สูงอายุ เกิดจากการเสื่อมถอยของหลายระบบทางสรีรวิทยาส่งผลให้ความสามารถในการปรับตัวลดลง การศึกษาเชิงพรรณนาแบบภาคตัดขวางครั้งนี้ใช้ทฤษฎีการปรับตัวของรอยเป็นกรอบแนวคิด เพื่อศึกษาอำนาจในการทำนายของการติดเชื้อระบบทางเดินปัสสาวะ คุณภาพการนอนหลับ โรคเรื้อรังร่วมหลายโรค และการใช้ยาหลายขนาน เป็นสิ่งเร้าตรงกระตุ้นการปรับตัวด้านสรีระ ส่งผลต่อกระบวนการปรับตัวต่อภาวะเปราะบางของผู้สูงอายุต่อมลูกหมากโต กลุ่มตัวอย่างเป็นผู้สูงอายุเพศชาย ที่มีอายุตั้งแต่ 60 ปี ขึ้นไป ได้รับการวินิจฉัยเป็นโรคต่อมลูกหมากโต มารับบริการที่โรงพยาบาลระดับตติยภูมิแห่งหนึ่ง ในกรุงเทพมหานคร จำนวน 265 ราย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ได้แก่ แบบสอบถามข้อมูลส่วนบุคคลของผู้สูงอายุ แบบบันทึกข้อมูลทางคลินิก แบบประเมินองค์ประกอบภาวะเปราะบาง และแบบประเมินคุณภาพการนอนหลับของพิตต์สเบิร์ก วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติ binary logistic regression analysis.
ผลการศึกษาพบว่า กลุ่มตัวอย่างผู้สูงอายุโรคต่อมลูกหมากโตมีอายุเฉลี่ย 72 ปี (SD = 23) กลุ่มตัวอย่างมีภาวะเปราะบาง ร้อยละ 68.3 การติดเชื้อระบบทางเดินปัสสาวะ คุณภาพการนอนหลับ โรคเรื้อรังร่วมหลายโรค การใช้ยาหลายขนาน สามารถร่วมทำนายภาวะเปราะบางของผู้สูงอายุโรคต่อมลูกหมากโตได้ ร้อยละ 30 อย่างมีนัยสำคัญทางสถิติ (Nagelkerke R2 = 0.305, Predictive correct = 76.2%, p < .05) ปัจจัยที่ศึกษาทั้งหมดสามารถทำนายภาวะเปราะบางได้ ได้แก่ คุณภาพการนอนหลับ (Adjusted OR = 6.498, 95% CI = 3.144–13.430, p < .001) การติดเชื้อระบบทางเดินปัสสาวะ (Adjusted OR = 3.356, 95% CI = 1.082–10.412, p = 0.036) โรคเรื้อรังร่วมหลายโรค (Adjusted OR = 2.729, 95% CI = 1.370–5.437, p = 0.004) และการใช้ยาหลายขนาน (Adjusted OR = 2.070, 95% CI = 1.038–4.131, p = 0.039)
ผลการศึกษาบ่งชี้ว่า พยาบาลควรส่งเสริมให้ผู้สูงอายุโรคต่อมลูกหมากโตมีความรู้ความเข้าใจเกี่ยวกับภาวะเปราะบาง การจัดการคุณภาพการนอนหลับ มีการคัดกรองกลุ่มเสี่ยงที่มีโรคเรื้อรังร่วมหลายโรค ป้องกันการติดเชื้อระบบทางเดินปัสสาวะ และเฝ้าระวังกลุ่มเสี่ยงที่มีการใช้ยาหลายขนาน เพื่อวางแผนป้องกันภาวะเปราะบางในผู้สูงอายุโรคต่อมลูกหมากโต
