บทคัดย่อ

ภาวะความรู้คิดบกพร่องเล็กน้อย เป็นภาวะที่พบได้บ่อยในผู้สูงอายุที่ป่วยด้วยโรคเบาหวานชนิดที่ 2 ผู้ป่วยกลุ่มนี้มีความเสี่ยงต่อการเสื่อมถอยด้านการรู้คิดเร็วกว่าปกติ จากความผิดปกติทางเมตาบอลิซึมเรื้อรัง ความเข้าใจปัจจัยที่ส่งผลต่อการเกิดภาวะความรู้คิดบกพร่องเล็กน้อยในกลุ่มนี้จึงมีความสำคัญอย่างยิ่งต่อการตรวจพบความผิดปกติแต่เนิ่น ๆ และการวางแผนป้องกันภาวะสมองเสื่อมในระยะยาว การวิจัยแบบความสัมพันธ์เชิงทำนายครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาความชุกและปัจจัยทำนายภาวะความรู้คิดบกพร่องเล็กน้อยในผู้สูงอายุโรคเบาหวานชนิดที่ 2 กลุ่มตัวอย่างจำนวน 160 ราย เป็นผู้มารับบริการที่โรงพยาบาลส่งเสริมสุขภาพประจำตำบล ในเขตอำเภอสันทราย จังหวัดเชียงใหม่ เครื่องมือที่ใช้ในการรวบรวมข้อมูล ประกอบด้วย (1) แบบบันทึกข้อมูลส่วนบุคคล (2) แบบประเมินภาวะความรู้คิดบกพร่องเล็กน้อย ฉบับภาษาไทย (3) แบบประเมินคุณภาพการนอนหลับ (4) แบบประเมินภาวะซึมเศร้า และ (5) การทดสอบการเดินในระยะ 5 เมตร วิเคราะห์ข้อมูลด้วยใช้สถิติเชิงพรรณนา สถิติ correlation analysis และ multiple regression analysis

ผลการวิจัยพบว่า
1. ผู้สูงอายุโรคเบาหวานชนิดที่ 2 มีภาวะความรู้คิดบกพร่องเล็กน้อย ร้อยละ 81.30 (M = 18.50, SD = 4.52) มีระดับน้ำตาลในเลือดอยู่ในระดับปกติ (M = 120.91, SD = 27.11) คะแนนคุณภาพการนอนหลับอยู่ในระดับที่ไม่ดี (M = 6.56, SD = 3.20) ภาวะซึมเศร้าเฉลี่ยอยู่ในระดับที่ไม่มีภาวะซึมเศร้า (M = 7.79, SD = 4.47) และการทำหน้าที่ของร่างกายอยู่ในระดับที่ไม่ดี (M = 11.36, SD = 2.55)
2. การทำหน้าที่ของร่างกายมีความสัมพันธ์เชิงลบกับภาวะความรู้คิดบกพร่องเล็กน้อยอย่างมีนัยสำคัญทางสถิติ (r = –.284, p < .01) และระดับน้ำตาลในเลือด คุณภาพการนอนหลับ และภาวะซึมเศร้าไม่พบความสัมพันธ์อย่างมีนัยสำคัญทางสถิติกับภาวะความรู้คิดบกพร่องเล็กน้อย
3. การทำหน้าที่ของร่างกายสามารถร่วมกันทำนายภาวะความรู้คิดบกพร่องเล็กน้อยในผู้สูงอายุโรคเบาหวานชนิดที่ 2 ได้ร้อยละ 75 โดยการทำหน้าที่ของร่างกายสามารถทำนายภาวะความรู้คิดบกพร่องเล็กน้อยในผู้สูงอายุโรคเบาหวานชนิดที่ 2 ได้มากที่สุด (β= –.284, p < .001)

จากผลการศึกษา ควรบูรณาการการควบคุมโรคเบาหวานร่วมกับการเฝ้าระวังและส่งเสริมสมรรถภาพทางกายในการดูแลระดับปฐมภูมิ เพื่อป้องกันหรือชะลอการเสื่อมถอยด้านการรู้คิดในผู้สูงอายุโรคเบาหวานชนิดที่ 2
