ผลของโปรแกรมส่งเสริมความเชื่อด้านสุขภาพโดยใช้แอปพลิเคชันไลน์ต่อพฤติกรรมการใช้ยา และระดับความดันโลหิตในผู้ป่วยหลังเกิดภาวะความดันโลหิตสูงเร่งด่วน

บทคัดย่อ

ภาวะความดันโลหิตสูงเร่งด่วน เป็นภาวะฉุกเฉินทางคลินิกที่พบได้บ่อยในผู้ป่วยโรคความดันโลหิตสูงที่ไม่สามารถควบคุมความดันโลหิตได้ การวิจัยเชิงทดลองนี้ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมส่งเสริมความเชื่อด้านสุขภาพโดยใช้แอปพลิเคชันไลน์ต่อพฤติกรรมการใช้ยา และระดับความดันโลหิตในผู้ป่วยหลังเกิดภาวะความดันโลหิตสูงเร่งด่วน กลุ่มตัวอย่างจำนวน 48 คน เป็นผู้ป่วยโรคความดันโลหิตสูงหลังเกิดภาวะความดันโลหิตสูงเร่งด่วน คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มอย่างง่าย และสุ่มตัวอย่างเข้าสู่กลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 24 คน กลุ่มทดลองได้รับการพยาบาลตามปกติร่วมกับโปรแกรมส่งเสริมความเชื่อด้านสุขภาพโดยใช้แอปพลิเค-ชันไลน์ ซึ่งพัฒนามาจากกรอบแนวคิดความเชื่อด้านสุขภาพของ โรเซนสต็อค โปรแกรมประกอบด้วย (1) ส่งเสริมความเชื่อด้านสุขภาพของบุคคลโดยการให้ความรู้ผ่านสื่อภาพอินโฟกราฟิกผ่านแอปพลิเคชันไลน์ (2) ส่งเสริมปัจจัยการกระตุ้นการปฏิบัติโดยการส่งข้อความแจ้งเตือนให้รับประทานยา สอบถามอุปสรรคในการใช้ยาโดยใช้แอปพลิเคชันไลน์ และติดตามสอบถามโดยใช้โทรศัพท์ โปรแกรมนี้ใช้ระยะเวลา 4 สัปดาห์ ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการรวบรวมข้อมูลประกอบด้วยแบบสอบถามข้อมูลส่วนบุคคล และแบบสอบถามพฤติกรรมการใช้ยา วิเคราะห์ข้อมูลโดยสถิติเชิงพรรณนา สถิติ independent t-test และ paired t-test

ผลการวิจัยพบว่า กลุ่มทดลองที่ได้รับโปรแกรมส่งเสริมความเชื่อด้านสุขภาพโดยใช้แอปพลิเคชันไลน์ เป็นระยะเวลา 4 สัปดาห์ มีค่าเฉลี่ยคะแนนพฤติกรรมการใช้ยาสูงกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติ (t = 12.62, p < .05) และสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (t = -5.52, p < .05) และกลุ่มทดลองมีค่าเฉลี่ยระดับความดันโลหิตซิสโตลิกและค่าเฉลี่ยระดับความดันโลหิตไดแอสโตลิก ต่ำกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติ (t = -11.61, p < .05; t = -6.24, p < .05) และค่าเฉลี่ยระดับความดันโลหิตซิสโตลิกและค่าเฉลี่ยระดับความดันโลหิตไดแอสโตลิก ต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (t = 2.53, p < .05; t = 1.85, p < .05)

ผลการวิจัยนี้แสดงให้เห็นว่า โปรแกรมส่งเสริมความเชื่อด้านสุขภาพโดยใช้แอปพลิเคชันไลน์ช่วยส่งเสริมพฤติกรรมการใช้ยาให้เหมาะสม และสามารถลดระดับความดันโลหิตได้ ดังนั้น ทีมสุขภาพจึงควรนำโปรแกรมนี้ไปประยุกต์ใช้กับผู้ป่วยโรคความดันโลหิตสูงหลังเกิดภาวะความดันโลหิตสูงเร่งด่วน เพื่อส่งเสริมให้ผู้ป่วยมีพฤติกรรมการใช้ยาที่เหมาะสมต่อไป
