ผลของการเตรียมผู้ป่วยเพื่อส่องกล้องสำไส้ใหญ่แบบผู้ป่วยนอก ศูนย์ส่องกล้อง โรงพยาบาลสุราษฎร์ธานี

ผลของการเตรียมผู้ป่วยเพื่อส่องกล้องสำไส้ใหญ่แบบผู้ป่วยนอก ศูนย์ส่องกล้อง โรงพยาบาลสุราษฎร์ธานีมีวัตถุประสงค์เพื่อศึกษาระดับความสะอาดของลำไส้ จากการใช้แนวปฏิบัติการเตรียมผู้ป่วยเพื่อส่องกล้องสำไส้ใหญ่ กลุ่มตัวอย่างคือผู้ป่วยจำนวน 80 ราย ที่เข้ารับการตรวจวินิจฉัยความผิดปกติของลำไส้ใหญ่ ด้วยวิธีการส่องกล้องตรวจลำไส้ใหญ่ เครื่องมือวิจัยประกอบด้วย 1) แนวปฏิบัติการเตรียมผู้ป่วยก่อนส่องกล้องสำไส้ใหญ่ ได้แก่ การให้คำแนะนำการเตรียมตัวของผู้ป่วยก่อนส่องกล้อง แผ่นพับการเตรียมตัวส่องกล้องลำไส้ใหญ่ และการโทรศัพท์ติดตามก่อนถึงวันนัด 3 วันและ 1 วัน 2) แบบรวบรวมข้อมูลการเตรียมตัวเพื่อส่องกล้องสำไส้ใหญ่ ได้แก่แบบบันทึกข้อมูลทั่วไป ข้อมูลการรับประทานอาหารและน้ำ แบบบันทึกการปฏิบัติตามแนวปฏิบัติการเตรียมลำไส้ ได้ค่าความเที่ยงแบบสังเกตร่วมเท่ากับ 0.9 และแบบบันทึกระดับความสะอาดของลำไส้ ของ Boston Bowel Preparation Scale ค่าความเที่ยงแบบสังเกตร่วมเท่ากับ 1 เครื่องมือผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 3 ท่าน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา วิเคราะห์ระดับความสะอาดของลำไส้กับผลของการเตรียมผู้ป่วยเพื่อส่องกล้องสำไส้ใหญ่ด้วยสถิติไคสแควร์
ผลการศึกษาพบว่า กลุ่มตัวอย่างรับประทานอาหารตามแนวปฏิบัติที่กำหนด เป็น 5 แบบ แบบที่ 1 (วันที่ 1-2 อาหารกากน้อย วันที่ 3 อาหารเหลวใส) ร้อยละ 86.25 โดยมีความสะอาดของลำไส้ระดับดี และดีมากร้อยละ 83.75 กลุ่มตัวอย่างที่ไม่ได้รับประทานอาหารตามแนวปฏิบัติที่กำหนดร้อยละ13.75 โดยรับประทานอาหาร ตามแบบที่ 2 (วันที่ 1อาหารกากน้อย วันที่ 2-3 อาหารเหลวใส) ร้อยละ 6.25 พบความสะอาดของลำไส้ร้อยละ 100 นอกเหนือจากนี้กลุ่มที่รับประทานอาหาร แบบอื่น ๆ ร้อยละ 7.50 พบว่ามีความสะอาดของลำไส้ และลำไส้ไม่สะอาดร้อยละ 50 เท่ากัน
สรุป การเตรียมความสะอาดลำไส้โดยการงดอาหารกากมาก 2 วัน และรับประทานอาหารเหลวใส 1 วันร่วมกับการรับประทานยาระบายก่อนการส่องกล้องมีระดับความสะอาดของลำไส้ มากกว่าการรับประทานอาหารรูปแบบอื่น
