Nursing Care of Uterine Fibroid Patients Undergoing Total Hysterectomy and Cervical Dissection with open Abdominal Surgery and Laparoscopic Surgery: 2 Case Studies.

Total hysterectomy and cervix removal is the most common surgery in gynecological diseases. Currently, surgery can be performed in different ways and have different pros and cons. The objective of this study was to compare problems and nursing activities in 2 uterine fibroid patients. Who underwent total hysterectomy and cervix with open abdominal surgery and laparoscopic incision the study was based on medical records of inpatients, outpatients, and operating room records, Interviews with patients and relatives, the results of the study found that
- The period before surgery both patients had the same surgical anxiety, although the type of surgery was different. The different problem was that Patient 1 had an anemia before surgery and needed to receive blood before surgery.
- During surgery, both patients had the same problems and needs, namely: 1) Risk of hypothermia during surgery. 2) Risk of infection during surgery 3) Risk of danger of blood loss However, it was found that the patients who underwent laparoscopic surgery had greater blood loss than those who underwent laparoscopic surgery. Due to the larger wound size, the chance of blood loss is higher. The different problems were: 1) Patients undergoing micro-incision laparoscopic surgery had complications from intra-abdominal CO2 gasification and required treatment with Atropine 0.6 mg; and 2) Small-incision laparoscopic patients. There is a risk of nerve injury from prolonged lying on the trestle.
- In the postoperative period, it was found that both patients had problems and their needs were: 1) Unwell due to wound pain. Patients who had abdominal surgery had more pain levels than those who had laparoscopic incision surgery because the incision was larger and the tissue was injured more. However, the patients who underwent laparoscopic surgery had discomfort from shoulder pain as a result of intra-abdominal carbon dioxide infusion. 2) That the recovery after surgery using a small incision laparoscopic surgery is faster than an open abdominal surgery
The results of this study can be used as a basis for improving the operating room service in order to operate more efficiently and reduce complications that may occur to patients.
