Nursing Care in Patients Who Receive Anesthesia for Percutaneous Nephrolithotomy : 2 case studies

การให้ยาระงับความรู้สึกเพื่อผ่าตัดนิ่วในไตโดยการเจาะรูส่องกล้องผ่านผิวหนัง เป็นการให้ยาระงับความรู้สึกแก่ผู้ป่วยแบบทั่วไปที่ยุ่งยากซับซ้อน อาจมีภาวะแทรกซ้อนเกิดขึ้นได้ในขณะผ่าตัดและต้องได้รับการแก้ไขภาวะแทรกซ้อนอย่างมีประสิทธิภาพทันที วิสัญญีพยาบาลมีบทบาทสำคัญต้องมีความรู้ความสามารถ มีทักษะและประสบการณ์การดูแลผู้ป่วยเป็นผู้แปลผลที่ได้ร่วมกับประเมินอาการแสดงของผู้ป่วยตั้งแต่การค้นหาปัญหาและร่วมแก้ไขกับทีมสุขภาพ ดังนั้นผู้ศึกษานำเสนอกรณีศึกษาผู้ป่วยที่เข้ารับการผ่าตัดนิ่วในไตโดยวิธีการเจาะรูส่องกล้องผ่านผิวหนังเปรียบเทียบ 2 ราย ซึ่งกรณีศึกษา 2 รายมีส่วนคล้ายกันในเรื่องการผ่าตัด การให้ยาระงับความรู้สึก และต่างกันในเรื่องภาวะแทรกซ้อน กรณีศึกษารายที่ 1 เกิด Rt. Hydrothorax ขณะทำผ่าตัดและได้รับการแก้ไขภาวะแทรกซ้อนทันทีอย่างปลอดภัย กรณีศึกษารายที่ 2 กรณีศึกษารายนี้การทำผ่าตัดปลอดภัยไม่เกิดภาวะแทรกซ้อนขณะการให้ยาระงับความรู้สึกและการทำผ่าตัด วัตถุประสงค์ในการศึกษาเพื่อศึกษาเปรียบเทียบการพยาบาลผู้ป่วยที่รับการให้ยาระงับความรู้สึกเพื่อผ่าตัดนิ่วในไตโดยการเจาะรูส่องกล้องผ่านผิวหนังกรณีศึกษา 2 ราย เพื่อให้ผู้ป่วยปลอดภัยจากภาวะแทรกซ้อนในการให้ยาระงับความรู้สึกและการผ่าตัดจนกระทั่งจำหน่ายออกจากโรงพยาบาลกลับไปใช้ชีวิตได้อย่างปกติ ซึ่งผู้ศึกษาได้วินิจฉัยการพยาบาลเป็น 3 ระยะ คือ
ระยะก่อนการให้ยาระงับความรู้สึก ระยะขณะการให้ยาระงับความรู้สึกและระยะหลังการให้ยาระงับความรู้สึก ผลการศึกษานี้สามารถเป็นแนวทางการวางแผนการพยาบาลการให้ยาระงับความรู้สึกผู้ป่วยเพื่อผ่าตัดนิ่วในไตโดยการเจาะรูส่องกล้องผ่านผิวหนังได้ สามารถดูแลผู้ป่วยได้อย่างมีประสิทธิภาพมากขึ้นและเป็นข้อมูลเพิ่มเติมในการศึกษาขั้นต่อไป
