Abstract

This research is a qualitative research by the process of  lesson learned which has the objectives 1) To Lessons learned from the success factors in the management of diabetes care services Health network, Sam Ngam District, Phichit Province, and 2) To offer feedbacks for developing the management of diabetes care services. Health Network, Sam Ngam District, Phichit Province. Informant was public health personnel, diabetes care team, Village health volunteers, Diabetes patients and caregivers and community leaders Sam Ngam District Health Network Phichit Province 83 people. Data were collected by focus group discussion, and observation without participation

Results suggest that Factors of success in the management of diabetes care management The health network in Sam Ngam District, Phichit Province consists of 1) is a network service arrangement by a multi-disciplinary team of sub-district health promoting hospitals near home 2) is consistent and continuity of network service arrangements that meet standards 3) have a system Work that focuses on community participation 4) There is a systematic work process with a work plan for diabetic patient care throughout the network. and operate according to the plan Including meetings to analyze the results of the work and follow up regularly. 5) There is a strong team of village health volunteers. Committed and determined to follow up 6) The health team has a process for empowering caregivers in diabetic families. Village health volunteers and community leaders continually The guidelines for the development of the management of diabetes care services. Health Network, Sam Ngam District, Phichit Province It should be supervised by a multidisciplinary team of service units near home continuously and consistently by arranging the appropriate workforce and complete and standardized service equipment. and the development of a technology system to connect the entire network to enable effective tracking of diabetic treatment information

Recommendations from the results of this research can be used to develop a care system for patients with other chronic diseases with similar contexts
