Development of Nursing Care Model for Pregnant Women with Pregnancy Induce Hypertension

Abstract

This research and development aims to develop a nursing model for pregnant women with pregnancy-induced hypertension (PIH), study outputs and outcomes of nursing model. Research participants include 128 pregnant women who were screened for risk factors for PIH, 19 registered nurses, 5 of whom work in the antenatal care unit of Krabi Hospital, and 14 who work in the network of Krabi Hospital from October 2022 to March 2023. The research process is divided into 3 phases: Phase 1: Situational study problems and nursing models for pregnant women with PIH. Phase 2: development of nursing care models for pregnant women with PIH, divided into 2 cycles: the first cycle involved design and trialing; the second cycle involved practicing in real situation. The research tool used a knowledge assessment form, self-care behaviors, mental health, and satisfaction questionnaires of pregnant women and registered nurses. Determine the content's validity and reliability. Quantitative data were analyzed by frequency distribution, percentage, mean, standard deviation, and paired t-test.

The results of the development of a nursing care model for pregnant women with PIH consisted of: 1)screening for risk factors for PIH 2)knowledge assessment, self-care behavior, and mental health assessment. 3)promoting knowledge of PIH according to specific teaching programs; and 4)continuing care is followed up by community nurses. The results after using the developed nursing care model showed that pregnant women with PIH had higher knowledge scores, a self-care behavior score, and mental health score that was significantly higher (p<.001). The rate of pre-eclampsia with severe features was 8.33%, the rate of preterm labor was 8.33%, no eclampsia and no maternal death. The budget allocated for maternity care has decreased. Women who were pregnant and registered nurses showed high levels of satisfaction with the model's use. The findings indicate that the development of a nursing model for pregnant women with PIH is a successful strategy that can be applied to develop a quality healthcare system.
