The Development of the Palliative Care System for End- of-Life Patients by using Tele-nursing, Ranong Hospital

Abstract

This study is the Research and development. The project aims to develop a palliative care system for Tele-nursing and the study on the outcome of the development of a palliative care system for Tele-nursing. Nursing has begun operations from June 2022 to August 2023. The operation consists of 3 phases: 1) Situation analysis 2) Designing and developing the system of palliative and end-of-life care services 3) Evaluation of the system of palliative and end-of-life care services system. Thirty healthcare officers of the interdisciplinary team and thirty-two palliative patients were recruited to participate. The research tool used a quality-of-life assessment form, anxiety and depression scale, system compliance of nurses, and satisfaction questionnaires of the palliative care for end- of-life patients and interdisciplinary teams, form for recording information on family meeting activities and advance care planning home visit the treatment cost and the duration of the service process.  Qualitative data were analyzed using content analysis. Quantitative data were analyzed by frequency, percentage, mean, and standard deviation including paired t-test p<.05 was considered a statistically significant difference.

The effects of developing the Tele-nursing system for palliative care on patients are as follows: 1) patients’ quality of life, which was increased significantly at p < .01.              2) Anxiety and depression significantly reduced at p<.01.  3) The satisfaction of patients with the Tele-nursing system was 4.37±.64, indicating at a high level 4) increase of family meetings and advanced care plan activities at 89.29 % 5) increase of frequent home visits for the patients based on the criteria at 81.55 % 6) The treatment cost that was decreased with statistical significance at p<.01. 7) The service process that was decreased with statistical significance at p<.01. The effects of developing the Tele-nursing system for palliative care on officers of interdisciplinary teams: 1) The nurses using the Tele-nursing system to follow up with the patients was at 100 percent. 2) The satisfaction of the interdisciplinary team was 4.16±.69, indication a high level. Our results indicated that the development of a Tele-nursing system for palliative care results in an increase in quality of patient life, reduces anxiety and depression, and reducing the cost of treatment and service duration. Therefore, the Tele-nursing system for palliative care should be supported as a policy used for palliative care for end-of-life patients
