Development of pain management model for patients undergoing lumbar surgery at King Narai Hospital

This study of research and development design (Research & Development) was aimed to develop pain management model as well as to study the efficacy of the model among patients undergoing lumbar surgery at King Narai Hospital. The samples included 45 registered nurses including 43 patients undergoing lumbar surgery in control group receiving usual care. The experimental group was 43 patients undergoing lumbar surgery receiving pain management model developed from situational analysis, evidence based practice of Rosswurm และ Larrabe, and Transition theory of Meleis, et al. consisted of music therapy for pain management, outcomes indicator of care and manual of nursing about pain management. The instruments were pain management model, assessment of conscious and pain score, post-operation of analgesic drugs, knowledge of pain management, post-operation of pain management methods, impact of pain to the holistic health status, questionnaire of nurses and patients’ satisfaction. Data were analyzed using descriptive statistic, Independent T-Test, Paired T-Test, Wilcoxson Signed Ranks test, and Mann-Whitney U – Test.
The pain management model was developed from evidence-based practice showed that the amount of post-operation of analgesics used in experimental group was significant lower than control group, In addition, pain management methods, patients’ satisfaction, impact of pain to holistic health status of experimental group compared with control groups were improve statistically significant at p<0.05. Furthermore, post-test study of nursing knowledge of pain management were highly statistically significant at p<0.05. In addition, nurses’ satisfaction of using the pain management model were at a highest level.
This study suggests that nurses should use model of pain management developed from evidence based practice to apply for patients undergoing lumbar surgery, and other post-operative patients and evaluate evidently holistic outcomes.
