The effects of self–management program of hypertensive patients with chronic kidney disease stage 3 on self –management behaviors and blood pressure

Abstract

         This Quasi-experimental research aimed to examine the effects of self–management program for hypertensive patients with chronic kidney disease stage 3 on self-management behaviors and blood pressure. The program was developed on a basis of self-management theory of Briesch & Chafouleas and Kanfer. Fifty participants were conveniently selected from hypertensive patients with chronic kidney disease stage 3 attending the Hypertension clinic at Damnoensaduak hospital, Ratchaburi province. They were divided into “the control” and “the experimental” group with 25 patients in each group. The participants in the control group received routine nursing care while those in the experimental group received the self-management program with routine nursing care. This program consisted of health education and self-management skill training individual by using audio visual, providing a manual on self-management of hypertensive patients with chronic kidney stage disease 3 and recording their own self-management behavior when they were home. Data were collected by personal data record and health information, self-management behavior questionnaires and monitoring blood pressure levels.. The data were analyzed by descriptive statistics, Chi-Square test, Paired t-test and Independent t-test.

         The results showed that after completion of the program, the experimental group had mean score of self -management behavior significantly higher than before receiving the program and higher than the control group (p<.05). The mean of blood pressure in the experimental group was significantly lower than before receiving the program and in the control group (p<.05).

            Based on the results, it is suggested that nurses should apply the self-management program for hypertensive patients with complications of chronic kidney disease continuously to decreases blood pressure and to improve and sustain their self-management in eating food, taking medication, having exercise, and managing stress.
