Evaluation of the Nursing Service System for Assisting Outpatients in Using Portable Cardiac Event Recorders at a University Hospital

Patients with cardiac arrhythmias often visit physicians after symptoms have subsided. Therefore, the diagnosis of arrhythmias requires specialized testing, typically using a portable cardiac event recorder, to capture abnormal heart rhythms during symptomatic episodes in daily life. Therefore, diagnosis frequently requires specialized testing using a cardiac event recorder. This descriptive research aimed to evaluate the effectiveness of the nursing service system in assisting patients with the use of a portable cardiac event recorder, based on the CIPP evaluation model by Stufflebeam. The sample consisted of 84 patients suspected of having arrhythmias. Data collection tools included a personal information questionnaire, a self-efficacy questionnaire regarding device usage, a social support questionnaire for family assistance, and an outcome evaluation form for device usage. Data were analyzed using descriptive statistics.
The results showed that the nursing service system, which included structural and procedural adjustments, promoted nurses’ knowledge in patient education and involved follow-up every 7 days for one month. This approach was found to be an appropriate model for nursing services. Patients’ outcomes revealed that the majority of participants were female with a mean age of 53.26 years. Approximately 74.80% had never used the device before, and 47.60% required assistance from family members. Moreover, 74.10% perceived themselves as capable of using the device. Common operational problems included pressing the recording button multiple times 23.80% and forgetting the usage steps 17.90%. The success rate of ECG transmission improved from 87.00% in the first week to 96.00% by the fourth week.
This study demonstrates that a nursing service system integrating patient education, consistent follow-up, and interdisciplinary collaboration can effectively support patients in using portable cardiac event recorders both efficiently and safely.
