บทคัดย่อ
Oroantral communication (OAC) เป็นภาวะแทรกซ้อนที่พบได้บ่อยจากการถอนฟันกรามน้อยบนและฟันกรามบน การเย็บปิดรูทะลุเป็นสิ่งที่สำคัญมากเมื่อรูทะลุมีขนาดมากกว่า 5 มิลลิเมตร เพื่อป้องกันการปนเปื้อนจากอาหารหรือน้ำลาย ซึ่งเป็นสาเหตุของการเกิดรูทะลุเชื่อมต่อถาวร (oroantral fistula) และโพรงอากาศอักเสบ (maxillary sinusitis) ได้ในภายหลัง รายงานนี้แสดงถึงการมีรากฟันเคลื่อนเข้าไปในโพรงอากาศระหว่างทำหัตถการ ทำให้เกิดรูทะลุเชื่อมต่อระหว่างโพรงอากาศแม็กซิลลากับช่องปาก โดยเน้นประเด็นไปที่การรักษา ซึ่งทำโดยการผ่าตัดเปิดเข้าไปในโพรงอากาศแม็กซิลลาโดยวิธีคาลด์เวล-ลุค (Caldwell-Luc) ซึ่งเป็นวิธีมาตรฐานในการนำเอาเศษรากฟันออกจากโพรงอากาศแม็กซิลลา ร่วมกับทำการผ่าตัดเลื่อนแผ่นเหงือกด้านกระพุ้งแก้ม (buccal advancement flap) มาเพื่อปิดรูทะลุ ภายหลังการรักษาพบว่า เนื้อเยื่อที่นำมาปิดมีการหายที่สมบูรณ์ดี ไม่พบอาการโพรงอากาศแม็กซิลลาอักเสบตามมา
