บทคัดย่อ
การพยุงชีพทางการแพทย์เป็นการรักษาที่นำมาใช้กับผู้ป่วยระยะวิกฤต เพื่อให้ผู้ป่วยมีชีวิตอยู่และได้รับการรักษาไปจนกระทั่งสามารถถอดถอนการพยุงชีพนั้นออกได้ในที่สุด ในกรณีที่ภาวะโรคนั้นไม่สามารถรักษาได้และอยู่ในระยะสุดท้ายของชีวิต การพยุงชีพเหล่านี้จะไม่เกิดประโยชน์ แต่จะสร้างความทุกข์ทรมานให้กับผู้ป่วย หากผู้ป่วยหรือญาติมีความประสงค์จะยุติการพยุงชีพเพื่อปลดเปลื้องความทุกข์ทรมาน หรือต้องการจะพาผู้ป่วยกลับไปเสียชีวิตที่บ้าน สามารถแจ้งให้ทีมสุขภาพดำเนินการถอดถอนการพยุงชีพนั้นได้อย่างถูกต้องตามกฎหมายที่กำหนดไว้ว่าบุคคลมีสิทธิทำหนังสือแสดงเจตนาไม่ประสงค์จะรับบริการสาธารณสุขที่เป็นไปเพียงเพื่อยืดการตายในวาระสุดท้ายของชีวิต
การถอดถอนการพยุงชีพในผู้ป่วยระยะสุดท้าย จำเป็นต้องสร้างความเข้าใจกับผู้ป่วยหรือญาติ รวมทั้งมีการสื่อสารอย่างถูกต้องด้วยความนุ่มนวลและความเห็นใจ โดยการให้ข้อมูลแก่ผู้ป่วยหรือญาติตามความเป็นจริง ร่วมกับดูแลให้ผู้ป่วยสุขสบาย ไม่ทุกข์ทรมาน อีกทั้งประคับประคองครอบครัวให้ผ่านพ้นวิกฤติและไม่ให้เกิดความรูสึกผิดกับการตัดสินใจนั้น ร่วมกับมีการประเมินและถอดบทเรียนในทีมสุขภาพที่เกี่ยวข้องเพื่อการพัฒนาการดูแลผู้ป่วยต่อไป
