Caring Falling Elderly Patients in Primary Care

Abstract
ความรู้ ความเข้าใจในปัญหาการหกล้มรวมถึงการให้ความสำคัญในผู้สูงอายุยังไม่เพียงพออยู่ค่อนข้างมาก ทั้งในส่วนของผู้สูงอายุและครอบครัว ชุมชนและสังคม รวมทั้งแพทย์และบุคลากรทางการแพทย์ การหกล้มมีทั้งเกิดการบาดเจ็บรุนแรงและไม่รุนแรง การหกล้มเปรียบได้กับสัญลักษณ์ที่บอกว่า ผู้สูงอายุรายนั้นน่าจะมีปัญหาภายในที่นำไปสู่การเสียชีวิตที่เพิ่มขึ้น ในความเป็นจริงมีผู้สูงอายุจำนวนมากที่หกล้มแล้วไม่ได้มารายงานให้แพทย์ทราบ โดยเฉพาะถ้าหกล้มนั้นไม่มีการบาดเจ็บเกิดขึ้นอย่างชัดเจน ขณะเดียวกันแพทย์ก็อาจจะไม่ได้ถามเรื่องการหกล้มของผู้ป่วยในระหว่างการติดตามการรักษา นอกจากนั้น แพทย์ที่ดูแลรักษาผู้ป่วยเมื่อมีการหกล้มและบาดเจ็บมักมุ่งให้ความสนใจเฉพาะเรื่องการบาดเจ็บจากการหกล้มและแก้ปัญหาเฉพาะหน้าที่เกิดขึ้น โดยไม่ได้มองหาสาเหตุที่ทำให้ผู้สูงอายุหกล้มเพื่อนำไปสู้การป้องกันการหกล้มที่อาจเกิดขึ้นได้อีก การค้นหาสาเหตุและวางแผนการป้องกันการหกล้มต้องอาศัยความเข้าใจในตัวผู้ป่วยสูงอายุและครอบครัวอย่างแท้จริง แพทย์ดูแลจึงสามารถสร้างที่จะวางแผนการช่วยเหลือได้อย่างถูกต้องและเหมาะสม
มีข้อมูลงานวิจัยทั้งในประเทศและต่างประเทศ กล่าวถึง ขบวนการบรรเทาปัญหาในหลายๆ ปัจจัย (Multi-factorial intervention) คือ การเข้าไปช่วยเหลือผู้สูงอายุและครอบครัวให้เกิดความจริง ทั้งในส่วนการลดยาระงับประสาทหรือยาอื่นๆ ที่ไม่จำเป็น การทำกายภาพบำบัดที่เหมาะสม และการเข้าไปปรับปรุงที่อยู่อาศัยในกลุ่มที่มีปัญหาในการเดินหรือการเคลื่อนไหวที่ผิดปกติ สามารถช่วยลดการหกล้มได้อย่างมีนัยสำคัญ
การเห็นผู้สูงอายุที่ล้มแล้วล้มอีกนั้น เห็นได้ว่าเป็นความทุกข์ระทมที่ผู้สูงอายุหลายคนเก็บไว้หรือแม้แต่บางครอบครัวรู้สึกว่าเป็นเรื่องที่ไม่รู้จะแก้ไขอย่างไร ซึ่งแท้จริงมีความสำคัญมากที่การแก้ปัญหานั้นทุกฝ่ายต้องช่วยกัน บอกเล่าเรื่องราวให้แพทย์ได้รับทราบ แพทย์และบุคลากรเห็นว่าต้องหาสาเหตุ และเข้าไปให้ความช่วยเหลือบรรเทาหรือแก้ไขปัจจัยต่างๆ ให้ดีขึ้นเห็นได้จริง ชุมชนและสังคมเห็นความสำคัญเช่นกัน ก็จะเกิดความต่อเนื่องในการสร้างเกิดการตระหนักในการดูแลผู้สูงอายุร่วมกัน เพื่อป้องกันการหกล้มเหมือนการป้องกันและเฝ้าระวังโรคหัวใจ โรคเส้นเลือดในสมอง ก็จะพบได้ว่า แท้จริงแล้วเรื่องนี้ทุกคนทำให้ปัญหาน้อยลงและบรรเทาลงได้
