การให้วัคซีนบาดทะยัก คอตีบ ไอกรน ในหญิงตั้งครรภ์

บทคัดย่อ
ถึงแม้วัคซีนบาดทะยักคอตีบไอกรนจะถูกรวมอยู่ในแผนการให้วัคซีนแห่งชาติที่มีมานานมากกว่า 40 ปี แต่อัตราการตายจากโรคทั้งสามยังไม่ลดลงโดยเฉพาะอย่างยิ่งในเด็กอายุ 0 ถึง 4 ปี นอกจากนี้อุบัติการณ์และอัตราตายจากโรคไอกรนพบว่าเพิ่มขึ้นในช่วงไม่กี่ปีที่ผ่านมา เพื่อแก้ปัญหาดังกล่าวจึงมีคําแนะนําจากหลายสถาบัน ให้มีการฉีดวัคซีนในหญิงตั้งครรภ์ด้วยวัคซีนบาดทะยักคอตีบไอกรน ในแต่ละการตั้งครรภ์ที่ระหว่างอายุครรภ์ 27 ถึง 36 สัปดาห์ ภูมิคุ้มกันที่มารดาสร้างขึ้นภูมิคุ้มจะถูกส่งผ่านไปยังทารกในครรภ์เพื่อป้องกันการติดเชื้อในเด็กก่อนถึงเวลาที่เด็กจะได้รับวัคซีนและสร้างภูมิคุ้มกันได้เองในช่วงหกเดือนแรกของชีวิต จากการทบทวนวรรณกรรมอย่างเป็นระบบ พบว่าประสิทธิภาพในการป้องกันโรคไอกรนในทารกอยู่ระหว่างร้อยละ 69-91 การป้องกันทารกนอนโรงพยาบาลจากโรคไอกรนอยู่ระหว่างร้อยละ 91-94 และการป้องกันทารกเสียชีวิตจากโรคไอกรนเท่ากับร้อยละ 95 ทั้งนี้ภาควิชาสูติศาสตร์-นรีเวชวิทยาคณะแพทยศาสตร์ศิริราชพยาบาล ได้เสนอแผนสําหรับการฉีดวัคซีนบาดทะยักคอตีบไอกรนในหญิงตั้งครรภ์ เพื่อความสะดวกในการวางแผนการฝากครรภ์กับบริบทของประเทศไทย ที่ผ่านมาหากหญิงตั้งครรภ์ไม่ได้รับการฉีดวัคซีนบาดทะยักกระตุ้นมานานมากกว่า 10 ปี จำเป็นจะต้องได้รับการฉีดกระตุ้นถึงสามเข็ม ทั้งนี้จากการทบทวนพบว่าการกระตุ้นเพียงเข็มเดียวด้วย dT หรือ Tdap ก็เพียงพอแล้ว ซึ่งแพทย์ผู้ดูแลจะต้องนัดหมายการฉีดวัคซีนเข็มกระตุ้นในกรณีที่ต้องการกระตุ้นด้วย Tdap ให้อยู่ในช่วงเวลาที่แนะนำ
