บทคัดย่อ
การให้บริการทางสุขภาพแก่คนข้ามเพศ ประกอบด้วย การยืนยันเพศสภาพด้วยวิธีทางสังคม การใช้ฮอร์โมนเพื่อการเปลี่ยนเพศและการผ่าตัดเพื่อการเปลี่ยนเพศ ผู้รับบริการที่เป็นคนข้ามเพศแต่ละรายอาจเลือกรับบริการแตกต่างกันออกไปตามความเหมาะสม โดยไม่จำเป็นต้องรับบริการครบทั้งสามส่วน การให้ฮอร์โมนเพื่อการเปลี่ยนเพศควรมีการพิจารณาเป็นรายๆไป โดยคำนึงถึงประโยชน์เมื่อเทียบกับความเสี่ยงทั้งในระยะสั้นและระยะยาวที่อาจเกิดขึ้น ความแตกต่างที่สำคัญระหว่างการใช้ฮอร์โมนข้ามเพศในหญิงและชายข้ามเพศคือ ในหญิงข้ามเพศวัยผู้ใหญ่ควรได้รับทั้งฮอร์โมนเอสโตรเจนและยาต้านฮอร์โมนแอนโดรเจน เพื่อเป็นการทดแทนฮอร์โมนเพศหญิงและกดฮอร์โมนเพศชายในร่างกายตามลำดับ ในอีกทางหนึ่ง หญิงข้ามเพศที่ผ่าตัดเปลี่ยนเพศที่ตัดอัณฑะออกแล้ว ควรใช้เพียงเอสโตรเจน ส่วนในชายข้ามเพศ การใช้ฮอร์โมนเทสโทสเตอโรนเพียงตัวเดียวสามารถทำหน้าได้ทั้งทดแทนและสามารถกดฮอร์โมนเดิมได้ด้วย โดยไม่ต้องคำนึงว่าได้ตัดต่อมเพศเดิมออกแล้วหรือไม่ การใช้ยาต้านไวรัสเพื่อป้องกันการติดเชื้อเอชไอวีในรูปแบบของ pre-exposure prophylaxis (PrEP) และ post-exposure prophylaxis (PEP) เป็นทางเลือกที่น่าสนใจเป็นอย่างมาก เนื่องจากมีประสิทธิภาพสูงในการลดการติดเชื้อ ในทางปฏิบัติ มีโอกาสที่คนข้ามเพศจะมีการใช้ฮอร์โมนเพื่อการเปลี่ยนเพศร่วมกับยาต้านไวรัสเอชไอวี การใช้ยาร่วมกันดังกล่าว ควรมีการคำนึงถึงโอกาสเกิดอันตรกิริยาระหว่างยาทั้งสองกลุ่ม การใช้ยาต้านไวรัสเอชไอวีอาจส่งผลต่อระดับฮอร์โมนในเลือดและยาฮอร์โมนก็ส่งผลต่อระดับของยาต้านไวรัสบางชนิด ในทางเวชปฏิบัติ การใช้ยาทั้งสองกลุ่มร่วมกันสามารถทำได้ โดยควรมีการเลือกชนิดของยา การตรวจติดตามระดับฮอร์โมนในเลือด และปรับขนาดของฮอร์โมนเพศให้เหมาะสม
