การเตรียมสไลด์ปัสสาวะด้วยเทคนิค Liquid-Based เพื่อการวินิจฉัยมะเร็งในระบบทางเดินปัสสาวะ

บทคัดย่อ
การตรวจเซลล์วิทยาจากปัสสาวะ (urine cytology) เป็นเครื่องมือสำคัญในการคัดกรอง วินิจฉัย และติดตามการกลับเป็นซ้ำของมะเร็งในระบบทางเดินปัสสาวะ โดยเฉพาะมะเร็งชนิด Urothelial Carcinoma อย่างไรก็ตาม วิธีการเตรียมสไลด์แบบดั้งเดิม (conventional smear) มีข้อจำกัดหลายประการที่อาจส่งผลกระทบต่อความแม่นยำในการวินิจฉัย เช่น การบดบังของเซลล์จากเมือก เลือด และการเกาะกลุ่มกันของเซลล์อย่างหนาแน่น รวมถึงการสูญเสียเซลล์ในขั้นตอนการเตรียมและการเสื่อมสภาพของเซลล์ ซึ่งทั้งหมดนี้อาจนำไปสู่ผลลบลวง (false-negative results) ได้ เทคนิคการเตรียมสไลด์แบบของเหลว (Liquid-Based Cytology: LBC) ได้รับการพัฒนาขึ้นเพื่อแก้ไขข้อจำกัดเหล่านี้ บทความนี้มีวัตถุประสงค์เพื่อทบทวนและวิเคราะห์หลักการทำงาน ข้อดีและประสิทธิภาพของเทคนิค LBC ในการประยุกต์ใช้กับการตรวจเซลล์วิทยาจากปัสสาวะ เทคนิค LBC ให้คุณภาพของสไลด์ที่ดีกว่าอย่างมีนัยสำคัญ โดยสามารถกำจัดสิ่งปนเปื้อนที่ไม่ต้องการและจัดเรียงเซลล์บนสไลด์เป็นชั้นบางๆ ที่สม่ำเสมอ ซึ่งเอื้อต่อการประเมินลักษณะทางสัณฐานวิทยาของเซลล์ โดยเฉพาะลักษณะสำคัญของนิวเคลียสได้อย่างชัดเจนและแม่นยำสูงขึ้น คุณสมบัตินี้มีความสำคัญอย่างยิ่งในการเพิ่มความไวของการตรวจพบมะเร็งชนิด High-Grade Urothelial Carcinoma ซึ่งเป็นชนิดที่มีความรุนแรงและต้องได้รับการวินิจฉัยอย่างรวดเร็ว อีกทั้งยังช่วยลดการวินิจฉัยในกลุ่มที่ผลไม่ชัดเจนได้อีก ดังนั้นการประยุกต์ใช้เทคนิค LBC ในงานเซลล์วิทยาของปัสสาวะมีบทบาทสำคัญในการเพิ่มคุณภาพของสไลด์และช่วยเพิ่มประสิทธิภาพการวินิจฉัยทางเซลล์วิทยานำไปสู่การวางแผนการรักษาผู้ป่วยที่แม่นยำและดียิ่งขึ้น
