การให้วัคซีนในหญิงตั้งครรภ์ ปี พ.ศ. 2569

บทคัดย่อ

การให้วัคซีนในหญิงตั้งครรภ์มีบทบาทสำคัญในการลดความรุนแรงของการติดเชื้อในหญิงตั้งครรภ์และทารกแรกเกิด โดยแอนติบอดีที่เกิดขึ้นสามารถถ่ายทอดผ่านรกไปยังทารกและให้การปกป้องในช่วง 6 เดือนแรกของชีวิต ประสิทธิภาพของการถ่ายทอดแอนติบอดีขึ้นกับระดับแอนติบอดีของมารดา ชนิดของวัคซีน และช่วงอายุครรภ์ที่ได้รับวัคซีน ซึ่งโดยทั่วไปต้องใช้เวลาอย่างน้อย 2–4 สัปดาห์ในการสร้างภูมิคุ้มกัน และอีกประมาณ 3-6 สัปดาห์ในการส่งผ่านสู่ทารก แอนติบอดีจะถูกส่งผ่านทางน้ำนมได้ในปริมาณน้อย ดังนั้น การให้วัคซีนในช่วงอายุครรภ์ที่เหมาะสมเป็นสิ่งที่สำคัญยิ่ง จากแนวทางการให้วัคซีนในหญิงตั้งครรภ์ ปี พ.ศ. 2568 วัคซีนที่แนะนำสำหรับหญิงตั้งครรภ์ 4 ชนิด ได้แก่ วัคซีนไข้หวัดใหญ่ วัคซีนโควิด-19 วัคซีนไอกรน และวัคซีน Respiratory syncytial virus (RSV) โดยมีช่วงอายุครรภ์ที่เหมาะสมแตกต่างกัน วัคซีนไข้หวัดใหญ่และโควิด-19 แนะนำตั้งแต่อายุครรภ์อย่างน้อย 12 สัปดาห์ วัคซีนไอกรนแนะนำช่วงอายุครรภ์ 20–32 สัปดาห์ และวัคซีน RSV แนะนำช่วงอายุครรภ์ 24–36 สัปดาห์ หลักฐานทางคลินิกสนับสนุนว่าวัคซีนเหล่านี้สามารถลดอัตราการป่วยรุนแรง การเข้ารับการรักษาในหอผู้ป่วยวิกฤต และการเสียชีวิตของทารกได้อย่างมีนัยสำคัญ ข้อมูลในปัจจุบันพบว่าผู้ป่วยโควิด-19 รายใหม่มีความรุนแรงของโรคลดลง ร่วมกับวัคซีนที่มีใช้อยู่ในปัจจุบันไม่สามารถป้องกันการติดเชื้อเนื่องจากมีการกลายพันธุ์ของเชื้ออย่างต่อเนื่อง ดังนั้น จึงควรพิจารณาให้วัคซีนโควิด-19 สำหรับผู้ที่มีความเสี่ยงสูงเท่านั้น และในแนวทางเวชปฏิบัติของราชวิทยาลัยสูตินรีแพทย์แห่งประเทศไทย ปี พ.ศ. 2569 ได้นำวัคซีนนี้ออกจากคำแนะนำสำหรับหญิงตั้งครรภ์ทั่วไป แม้ว่าวัคซีนบางชนิดยังมีข้อจำกัดด้านค่าใช้จ่ายและการเข้าถึง การบูรณาการการให้วัคซีนเข้าสู่ระบบการฝากครรภ์ตามปกติ และการสื่อสารอย่างกระจ่างชัดระหว่างบุคลากรทางการแพทย์ หญิงตั้งครรภ์ และครอบครัวของหญิงตั้งครรภ์ ในเรื่องประสิทธิภาพและความปลอดภัยของวัคซีนแต่ละชนิด ในแต่ละบริบทอย่างเหมาะสม จะเป็นแนวทางในการป้องกันโรคติดเชื้อระบบทางเดินหายใจชนิดรุนแรงในหญิงตั้งครรภ์และทารกแรกเกิดอย่างยั่งยืน
