แนวทางการออกกำลังกายในผู้ป่วยปวดหลังส่วนล่างเรื้อรังแบบไม่จำเพาะเจาะจง: การสังเคราะห์หลักฐานเชิงประจักษ์และการประยุกต์ใช้ในชุมชน

บทคัดย่อ
ปวดหลังส่วนล่างเรื้อรังแบบไม่จำเพาะเจาะจงเป็นปัญหาสุขภาพสำคัญที่ส่งผลต่อความพิการและภาระทางเศรษฐกิจในระดับประชากร แนวทางสากลแนะนำให้การออกกำลังกายเป็นการรักษาลำดับแรก อย่างไรก็ตาม แนวทางเชิงปฏิบัติสำหรับการออกกำลังกายด้วยตนเองที่บ้านในบริบทชุมชนยังขาดความชัดเจน บทความนี้เป็นการทบทวนวรรณกรรมเชิงบรรยาย (narrative synthesis) มีวัตถุประสงค์เพื่อสังเคราะห์หลักฐานเชิงประจักษ์เกี่ยวกับการออกกำลังกายในผู้ป่วยปวดหลังส่วนล่างเรื้อรังแบบไม่จำเพาะเจาะจง และพัฒนาแนวทางการออกกำลังกายที่บ้าน พร้อมเสนอกรอบการประยุกต์ใช้ในระดับชุมชน การศึกษานี้สืบค้นข้อมูลจากฐานข้อมูล PubMed, Scopus, Cochrane Library และ PEDro ครอบคลุมการทดลองแบบสุ่มมีกลุ่มควบคุม การทบทวนเชิงระบบ และการวิเคราะห์อภิมานในช่วงปี ค.ศ. 2015–2025 โดยคัดเลือกงานวิจัยที่เกี่ยวข้องกับการออกกำลังกายในผู้ป่วยปวดหลังส่วนล่างเรื้อรังแบบไม่จำเพาะเจาะจง
ผลการสังเคราะห์พบว่า การออกกำลังกายสามารถลดอาการปวดและความพิการได้ในระดับเล็กถึงปานกลาง มีความปลอดภัย และเหมาะสมเป็นการรักษาลำดับแรก โดยไม่พบรูปแบบการออกกำลังกายใดที่เหนือกว่าอย่างชัดเจน โปรแกรมแบบผสมผสานที่รวมการฝึกความมั่นคงของแกนกลาง การเพิ่มความยืดหยุ่น และกิจกรรมแอโรบิกให้ผลลัพธ์ที่เหมาะสม ผลการสังเคราะห์ยังสะท้อนให้เห็นรูปแบบโปรแกรมการออกกำลังกายที่หลากหลาย และนำไปสู่การพัฒนาแนวทางการออกกำลังกายที่บ้านระยะเวลา 12 สัปดาห์ ภายใต้หลักการเพิ่มระดับอย่างค่อยเป็นค่อยไป การติดตามระดับความปวด และการส่งเสริมการกำกับตนเอง พร้อมทั้งเสนอรูปแบบการนำไปใช้จริงในชุมชนซึ่งผสมผสานองค์ประกอบ 4 ด้าน ได้แก่ ความสามารถทางกายภาพ การควบคุมระบบประสาทและกล้ามเนื้อ การปรับ เปลี่ยนทางจิตสังคม และการควบคุมพฤติกรรมตนเอง โดยสรุป การออกกำลังกายที่บ้านโดยอิงตามกรอบแนวคิดชีวจิตสังคม มีศักยภาพสูงในการลดภาระของระบบสุขภาพและส่งเสริมการจัดการตนเองอย่างยั่งยืนในของชุมชนบริบท
