บันทึกทางการพยาบาล : หลักการบันทึกเพื่อการใช้ประโยชน์ทางกฎหมาย

Abstract

บันทึกทางการพยาบาล เป็นข้อความการพยาบาลต่อผู้รับบริการที่พยาบาลเป็นผู้บันทึกไว้ โดยมีข้อมูลเกี่ยวกับอาการและอาการแสดงของผู้รับบริการ ข้อวินิจฉัยทางการพยาบาล การปฏิบัติกิจกรรมพยาบาล และผลลัพธ์ ดังนั้นนอกจากการบันทึกทางพยาบาลจะเป็นสิ่งสะท้อนองค์ความรู้และการปฏิบัติงานของพยาบาลแล้วยังใช้เป็นหลักฐานทางกฎหมายที่มีความสำคัญยิ่งเมื่อมีการฟ้องร้องเกิดขึ้น การเขียนบันทึกทางการพยาบาลให้มีคุณภาพจึงเป็นสมรรถนะที่จำเป็นที่พยาบาลจะต้องให้ความสำคัญ และจะต้องบันทึกอย่างมีคุณภาพในการทำงานทุกเวร ทุกวัน อย่างต่อเนื่อง

Nurses’ note : the principles for utilization in legislation

Nurses’ note is a nursing message for patients that is recorded by their nurses. It provides the information about a patient’s signs and symptoms, nursing diagnoses, nursing practices and results. Nurses’ note not only reflects the nurse’s knowledge and performance for the patient’s benefit, but may also serve as legal evidence which is very important. When  lawsuits or legal prosecution occur; thus, writing a high quality nurses’ note is a necessity that nurses have to pay attention to. Nurses must consistently record it with the highest level of dilligence every work shift of every single day.
