บทบาทพยาบาล: การป้องกันการพลัดตกหกล้มในผู้ป่วยจิตเวช

Abstract

การพลัดตกหกล้มในผู้ป่วยจิตเวชที่รับไว้รักษาในโรงพยาบาล พบว่ามีอุบัติการณ์ในหอผู้ป่วยจิตเวช สูงกว่าหอผู้ป่วยวิกฤตทั่วไป การพลัดตกหกล้มในผู้ป่วยจิตเวชเกิดจากหลายปัจจัย ประกอบด้วย พยาธิสภาพของโรค ผลข้างเคียงของยาทางจิตเวช การรักษาด้วยไฟฟ้า และกิจกรรมบำบัดที่กระตุ้นให้ผู้ป่วยมีกิจกรรมตลอดทั้งวัน จากการทบทวนวรรณกรรมการพลัดตกหกล้มในผู้ป่วยจิตเวชพบว่าอุบัติการณ์การพลัดตกหกล้ม มากกว่าครึ่งหนึ่งสามารถป้องกันได้ หากพยาบาลประเมินความเสี่ยงของผู้ป่วยแต่ละรายได้อย่างถูกต้อง และให้การดูแลป้องกันความเสี่ยงที่เหมาะสมและสอดคล้องกับบริบทที่แท้จริงของกลุ่มผู้ป่วยจิตเวช รวมทั้งการสร้างวัฒนธรรมองค์กรแห่งความปลอดภัยเพื่อให้บุคลากรตระหนักถึงความสำคัญและมีส่วนร่วมในการป้องกันการพลัดตกหกล้มในผู้ป่วยจิตเวช  บทความนี้จะนำเสนอ ความหมายและประเภทของการพลัดตกหกล้ม ปัจจัยเสี่ยงที่สัมพันธ์กับการพลัดตกหกล้มในผู้ป่วยจิตเวชและบทบาทของพยาบาลในการป้องกันการพลัดตกหกล้มในผู้ป่วยจิตเวช 

Nurse’s role: fall prevention among psychiatric patients

Incidences of falls among psychiatric patients found in psychiatric units tends to be higher than that of general acute care hospital units. The high incidence of falls is attributed to many factors such as psychiatric symptoms, side effects of psychiatric medication, electroconvulsive therapy (ECT) and activity therapy. From a review literature about the falls of psychiatric patients it was found that the incidence of falls can be prevented if the nurse can do a risk assessment of inpatients, provide appropriate supervision and risk prevention appropriate to the actual context of the psychiatric inpatient. As well as the development of patient safety culture aiming to increase the concerns of patient safety among health care providers, this article presents the risk factors associated with falls in psychiatric inpatients and nurse's roles in fall prevention in psychiatric inpatients.
