Abstract

Hip fracture in elderly is one of the important problems that needs hip arthroplasty correction. Nowadays hip arthroplasty is good method for safety and ease to post-operative rehabilitation. Scrub nurse had role in nursing care process for prevent complication and early rehabilitation phase. The objective of this comparative case study was compared perioperative nursing care 2 cases of hip arthroplasty with compared the demographic data, nursing diagnosis and nursing care. Method: Two elderly female with comorbid disease who undergone hip arthroplasty surgery in operating room, Sakonnakhon hospital between November 2021 and May 2022. The data was collected with conceptual framework of Gordon’s 11 functional health patterns and nursing process caring.

The results showed that general data, causes, signs and symptoms, diagnosis had no different between 2 cases, but the progression was different. Case study 1 The operation was bipolar cementless hemiarthroplasty left femur under general anesthesia. Case study 2 had more risk factors as ischemic stroke and poor control blood pressure level. The operation was bipolar cementless hemiarthroplasty right femur under spinal block. Intraoperation revealed that the both case study had 19 similar nursing diagnosis and 2 different diagnosis. Post operation both patient were safety and had no complication.

Conclusion and recommendations: Both case study were elderly with comorbidities undergone to surgery of hip arthroplasty, that were high risk for operation. It was necessary to require systemic caring by the multidisciplinary team. Therefore, perioperative nurses and health care team need high competency specific clinical nursing practice guideline in perioperative nursing care of hip arthroplasty for safety and good results.
