Effects of the Individual and Family Self-Management Program on Bowel Preparation Behavior for Colonoscopy at Udon Thani Hospital

This research was a pre-experimental two groups posttest-only design. The objectives were to compare bowel preparation behavior for colonoscopy and the difference in colon cleanliness in patients between the experimental group who received the individual and family self-management program and the control group who received conventional care. The participants were outpatients who should be conducted first time colonoscopy during April - August 2021. Participants who met the inclusion criteria were divided into experimental group and control group, 36 each, matched for sex, age and type of laxative. The experimental group received an individual and family self-management program based on Ryan and Sawin's theory. The patients’ families took part in providing care and assistance to patients in 5 steps; goal setting, reflection, action planning, decision making and evaluation. The research tools were: 1) Individual and Family Self-Management Programs consist of a colonoscopy practice guide and video via Line application. 2) the data collection tools consisted of questionnaire for personal data and health status of the patients and caregivers for behavioral assessment of bowel preparation for a colonoscopy. 3) The knowledge scale assessment; patients and primary caregivers in experiment group were qualified by passed 80% or higher score. The investigator used the colon cleanliness assessment scale according to the Aronchick Bowel Preparation Scale (ABPS). The instrument was validated by five experts. The reliability of the questionnaires were 1.0, 0.85, 0.80, 0.80, and 0.85 for the personal data and health status of the patients; the personal data of the main caregiver; a questionnaire for behavioral assessment of bowel preparation for a colonoscopy; the consistence of a colonoscopy practice guide; and video topic via the Colonoscopy group in Line application. Reliability of the assessment form for bowel preparation behavior for colonoscopy is 0.70. Descriptive statistics was performed, inferential using the Mann-Whitney U Test and Chi-Square Test.
The results showed most of them were female (58.3%), mean age 59.3 years old. The colonoscopy preparatory behavior of patients undergoing colonoscopy in the experimental group who receiving the individual and family self-management program was good level in all items compared to the control group receiving conventional care was fair level, this had significant difference (p<0.05). Colon cleanliness in the experimental group was clean (97.2%) better than in the control group (clean 30.6%), at p<0.001.
Conclusion: the individual and family self-management program affects the bowel preparation behavior scores for a better colonoscopy cleanliness.
