Prevalence and Risk Factors for Colistin-resistant Acinetobacter baumannii Infection among Patients in Udon Thani Hospital

Colistin-resistant Acinetobacter baumannii is currently the extensive drug-resistant organism causing nosocomial infection. This study aimed to identify the prevalence and risk factors for colistin-resistant A. baumannii infection among patients in Udon Thani hospital. This single-center retrospective study was conducted at Udon Thani Hospital, Thailand, from January 2018 to December 2020. We analyzed 140 patients with non-colistin-resistant A. baumannii (non-CoR-AB) infection and 20 patients with colistin-resistant A. baumannii (CoR-AB) infection. Data analysis using descriptive statistics such as frequency, percentage, mean and standard deviation. Inferential Statistics for between-group comparisons using Fisher's exact test and Student's t-test. Risk Factors for CoR-AB infection were analyzed using univariable logistic regression and multivariable logistic regression, presenting Adjusted Odds Ratio (AOR), and 95% confidence interval (95% CI) at a significant level of p<0.05.
Results: The prevalence of CoR-AB infection was 0.69% (20/2,907). Univariable analysis was performed to evaluate the risk factors for CoR-AB infection. CKD (OR 3.64, 95% CI 1.00-13.18, p = 0.05), Duration of clindamycin (OR 1.18, 95% CI 1.03-1.34, p = 0.02), Duration of ceftazidime (OR 1.12, 95% CI 1.01-1.23, p = 0.03), Duration of mechanical ventilator more than 14 days (OR 3.24, 95% CI 1.24-8.49, p = 0.02), Duration of foley catheterization more than 30 days (OR 3.23, 95% CI 1.15-9.08, p = 0.03), Duration of hospital stay before infection (OR 1.08, 95% CI 1.02-1.14, p = 0.01) were associated with CoR-AB infection. But the two groups showed no significant differences in multivariable analysis.
Conclusion: From January 2018 to December 2020, the prevalence of CoR-AB infection in UTH was 0.69% (20/2,907). This study could not identify significant risk factors for CoR-AB infection. Data about the risk factors of the resistant pathogen is crucial for appropriate standard of care that prevents the infection caused by super bug.
