Abstract
Preterm infants often have immature coordination of sucking, swallowing, and breathing, resulting in delayed oral feeding onset, dependence on tube feeding, and prolonged hospitalization. This study aimed to synthesize empirical evidence on patterns of maternal and family involvement, related factors, and outcomes concerning sucking/feeding, development, and clinical status among preterm infants. This integrative review was conducted using Whittemore and Knafl's framework in conjunction with PRISMA guidelines, searching studies published from 2017–2026 across the CINAHL, MEDLINE Complete, Academic Search Ultimate, PsycINFO, and PubMed databases. Study quality was appraised using the Mixed Methods Appraisal Tool (MMAT 2018), and data were synthesized thematically.
Fifteen studies meeting the eligibility criteria were identified from 255 records. The patterns of involvement identified included non-nutritive sucking, oral stimulation, breast stimulation, and kangaroo care, supported by facilitating factors such as knowledge acquisition, emotional support, and family-centered care. Five outcome themes were synthesized: 1) non-nutritive sucking and oral stimulation promote feeding readiness; 2) breast stimulation and lactation support promote breastfeeding; 3) kangaroo care and positive touch support physiological stability and development; 4) education and family-centered care facilitate family involvement; and 5) success evaluation should encompass feeding, neurobehavioral, clinical, and family outcomes. Direct evidence indicated improvements in sucking behavior and breastfeeding, while contextual evidence contributed to strengthening caregiver bonding and confidence.
Maternal and family involvement is a key component of promoting feeding and developmental care among preterm infants. Implementation should begin with assessing infant readiness, providing education and hands-on training for families, supporting breastfeeding, and maintaining systematic safety monitoring.
