Prevalence of Potentially Inappropriate Medications and Polypharmacy in Geriatric Clinic of Cham Phak Phaeo Subdistrict Health Promotion Hospital, Kaeng Khoi District, Saraburi Province during 1 September 2022–31 August 2023

Objective: To study prevalence and factors associated with potentially inappropriate medications using the AGS 2023 Beers criteria and the prevalence of polypharmacy of geriatric clinic at Cham Phak Phaeo Subdistrict Health Promoting Hospital, Kaeng Khoi district, Saraburi.
Method: Retrospective cross-sectional study conducted in 343 elderlies aged 60 and above who underwent geriatric screening at Cham Phak Phaeo Subdistrict Health Promoting Hospital from September 1, 2022, to August 31, 2023. The study included those who had at least one prescription and creatinine data during their outpatient visits
Results: Among of 343 elderly people, 147 of them (42.86%) had inappropriate medication use according to AGS 2023 Beers criteria and 223 of them (65.01%) had polypharmacy, using five or more medications. Factors associated with inappropriate medication included polypharmacy, female gender, and chronic diseases in the bone and joint, muscle and tendon, and neuropsychiatric categories. Top three medication classes were skeletal muscle relaxants, central nervous system drugs, and antihistamine drugs and five most common medications associated with inappropriate uses were Dimenhydrinate, Chlorpheniramine, Tolperisone, Lorazepam, and NSAIDs respectively.
Conclusions: Some of the health facilities are still lack of alternative medications for Dimenhydrinate, Chlorpheniramine, Lorazepam and Amitriptyline, which are commonly prescribed for the elderly. Recommendations include reviewing medication procurement practices and enhancing caution in prescribing for the elderly.
