บทคัดย่อ
การวิิจััยเชิิงพรรณนานี้มีีวัตถุุประสงค์เพื่่อศึึกษาบทบาทการดููแลแบบประคัับประคองของพยาบาลในหน่่วยบริิการปฐมภููมิิ ซึ่งเป็็นส่วนหนึ่งของวิิจััยสภาการพยาบาล เรื่องบทบาทความรัับผิิดชอบของพยาบาลในการขัับเคลื่อนระบบบริิการสุุขภาพระดัับปฐมภููมิิ กลุ่มตัวอย่างเป็็นพยาบาลปฏิิบััติิงานดููแลแบบประคัับประคอง ในหน่วยบริิการปฐมภููมิิ เขตสุุขภาพที่่ 11 จำนวน 33 ราย โดยการคััดเลืือกแบบหลายขั้นตอน เครื่องมืือที่่ใช้คืือ แบบสอบถามบทบาทการดููแลแบบประคัับประคองของพยาบาล แนวคำถามกระบวนการดููแลแบบประคัับประคองและปัญหาอุุปสรรคในการทำงาน วิิเคราะห์ข้อมููลโดยใช้สถิิติิพรรณนา Mann-Whitney U Test และวิิเคราะห์เชิิงเนื้อหา ผลการวิิจััยพบว่า พยาบาลที่่โรงพยาบาลส่งเสริิมสุขภาพตำบลปฏิิบััติิงานในบทบาทการดููแลแบบประคัับประคองภาพรวมมากกว่าพยาบาลที่่คลินิิกหมอครอบครัวอย่างมีีนัยสำคัญทางสถิิติิ โดยเฉพาะบทบาทการสร้างเครืือข่ายการดููแลในชุุมชน การให้ความรู้และฝึึกทัักษะการดููแลแก่ผู้ดููแลและญาติิ การจััดการ/ช่วยบรรเทาอาการปวด และการประสานงานและดููแลเมื่่อเข้าสู่วาระสุุดท้ายตามลำดับ สำหรัับกระบวนการดููแล ประกอบด้วย การเตรีียมการการดููแลที่่บ้านการสร้างเครืือข่ายการดููแล การพััฒนาศัักยภาพผู้ดููแล และการติิดตามหลัังเสีียชีีวิิต และปัญหาอุุปสรรคต่อการปฏิิบััติิงานของพยาบาลทั้งสองแห่งคืือ ขาดนโยบายการดููแลแบบประคัับประคองระดัับปฐมภููมิิที่่ชััดเจน ขาดการพััฒนาความรู้และอุุปกรณ์ที่่จำเป็็น มีีภาระงานมาก และบุุคลากรไม่เพีียงพอ ข้อเสนอแนะ คืือ 1) สนัับสนุุนนโยบายการดููแลแบบประคัับประคองในชุุมชนที่่ชััดเจนทั้งระดัับตำบล อำเภอ และจัังหวััด และขัับเคลื่อนสู่การปฏิิบััติิให้เป็นรููปธรรม 2) ให้การสนัับสนุุนความรู้โดยการอบรมและอุุปกรณ์ที่่จำเป็นให้เพีียงพอ และ3) กระทรวงสาธารณสุุขควรทบทวนกรอบอััตรากำลังของพยาบาลให้เพีียงพอและสอดคล้องกัับภาระงาน
