ผลของการใช้แนวปฏิบัติการพยาบาลเพื่อส่งเสริมผู้ดูแลในการจัดการดูแลต่อพฤติกรรมการดูแลสุขภาพและผลข้างเคียงระยะยาว ในเด็กโรคมะเร็งเม็ดเลือดขาวเฉียบพลันชนิดลิมโฟบลาสต์ภายหลังสิ้นสุดการรักษา

บทคัดย่อ
การศึกษาครั้งนี้มีวัตถุุประสงค์พัฒนาแนวปฏิบััติการพยาบาลเพื่อส่งเสริมผู้ดูแลในการจัด การดููแลเด็กโรคมะเร็งเม็ดเลือดขาวเฉียบพลัน ชนิดลิมโฟบลาสต์ภายหลังสิ้นสุดการรักษา และนำมาทดลองใช้โดยเปรียบเทียบพฤติกรรมการดูแลสุุขภาพเด็กโรคมะเร็งเม็ดเลือดขาวเฉียบพลัน ชนิดลิมโฟบลาสต์ ก่อนและหลังได้รับการสนับสนุุนการจัด การดูแลเด็ก และติดตามผลข้างเคียงระยะยาวภายหลังสิ้นสุุดการรักษา
กลุ่มตัวอย่างคือผู้ดููแลเด็กโรคมะเร็งเม็ดเลือดขาวเฉียบพลันชนิดลิมโฟบลาสต์ตั้งแต่อายุุ 1 ปี ถึง 12 ปีจำนวน 20 ราย ได้รับการสนับสนุนตามแนวปฏิบัติการพยาบาลที่พัฒนาจากแนวคิดการจัดการตนเองของเครียร์ ประกอบด้วยการตั้งเป้าหมาย การเก็บรวบรวมข้อมููล การประมวลผลและการประเมินผลการตัดสินใจ การลงมือปฏิบัติและการสะท้อนกลับ ตนเอง แบ่งออกเป็น 3 ระยะ ได้แก่ ระยะเตรียมการ ระยะดำเนินการ และระยะติดตาม รวมเวลา 4 เดือน เครื่องมือที่ช้ในการเก็บรวบรวมข้อมููล คือ แบบบันทึกข้อมููลส่วนบุคคล (สำหรับ ผู้ดูแล) แบบ ประเมินผ ลข้างเคี ยงระยะยาว (สำหรับ ผู้ดููแล) และแบบสอบถามพฤติกรรมดูแลสุุขภาพเด็ก วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและสถิติควาแปรปรวนทางเดียวแบบวัดซ้ำผลการศึกษาพบว่า ภายหลังได้รับ การสนับสนุุนตามแนวปฏิบัติ การพยาบาล กลุ่มตัว อย่างมีคะแนนเฉลี่ยพฤติกรรมการดูแลสุขภาพเด็กโรคมะเร็งเม็ดเลือดขาวเฉียบพลัน ชนิดลิมโฟบลาสต์เพิ่มสูงขึ้นอย่างมีนัยสำคัญทางสถิติ หลังดำเนินการ 1 เดือน พบมีอาการซีด 2 ราย หลังดำเนินการ 4 เดือน เด็กทั้งหมดไม่พบอาการหรืออาการแสดงความผิดปกติในทุกระบบจ ากผลการศึกษาแสดงให้เห็นว่า พยาบาลสามารถนำแนวปฏิบัติการพยาบาลไปใช้กับ ผู้ดูแลเด็กโรคมะเร็งเม็ดเลือดขาวเฉียบพลัน ชนิดลิมโฟบลาสต์เพื่อปรับเปลี่ยนพฤติกรรมการดูแลสุขภาพเด็กภายหลังสิ้นสุดการรักษาให้ เหมาะสมเพื่อส่งเสริมให้ มีสุขภาพดีขึ้น
