การใช้ยาต้านโคลิเนอร์จิก อาการไม่พึงประสงค์ และความรู้ในการใช้ยาของผู้สูงอายุ

บทคัดย่อ

 การเปลี่ยนแปลงทางสรีรวิทยาและพยาธิสภาพตามกระบวนการสูงอายุส่งผลกระทบโดยตรงต่อเภสัชจลนศาสตร์ทั้งในด้านการดูดซึม การกระจายตัว กระบวนการเปลี่ยนสภาพยาที่ตับ และการขับยาออกทางไต รวมถึงเภสัชพลศาสตร์ที่ทำให้ความไวต่อการตอบสนองต่อยาเพิ่มขึ้น ความซับซ้อนดังกล่าวเพิ่มความเสี่ยงต่อการเกิดอาการไม่พึงประสงค์จากการใช้ยาในผู้สูงอายุ โดยเฉพาะยากลุ่มต้านโคลิเนอร์จิกซึ่งใช้แพร่หลายในโรคเรื้อรัง เช่น โรคพาร์กินสัน ความดันโลหิตสูง และภาวะซึมเศร้า แม้จะมีประสิทธิภาพในการรักษา แต่การยับยั้งสารสื่อประสาทอะเซทิลโคลีนอาจก่อให้เกิดกลุ่มอาการอันไม่พึงประสงค์ที่รุนแรง เช่น ความบกพร่องด้านความจำและการรู้คิด ภาวะสับสน เฉียบพลันและปัสสาวะคั่ง ซึ่งส่งผลกระทบเชิงลบต่ออัตราการเข้าพักรักษาในโรงพยาบาลและการเสียชีวิต นอกจากนี้การได้รับยาหลายขนานในผู้สูงอายุที่มีโรคประจำตัวร่วมหลายโรค ยังเพิ่มความเสี่ยงต่อปฏิกิริยาระหว่างยาและผลข้างเคียงที่สะสม อย่างไรก็ตาม งานวิจัยเกี่ยวกับการใช้ยาต้านโคลิเนอร์จิกของผู้สูงอายุในปัจจุบันส่วนใหญ่มักจำกัดอยู่ในบริบทต่างประเทศ ซึ่งอาจไม่สอดคล้องกับบริบทสาธารณสุขไทยเนื่องจากความแตกต่างด้านพันธุกรรม รูปแบบการใช้ชีวิต และระบบสนับสนุนทางสังคม โดยเฉพาะพฤติกรรมการซื้อยาใช้เองและการใช้สมุนไพรที่อาจส่งเสริมฤทธิ์ต้านโคลิเนอร์จิกโดยไม่ตั้งใจ การศึกษานี้จึงมีวัตถุประสงค์เพื่อสำรวจพฤติกรรมการใช้ยา อาการไม่พึงประสงค์จากการใช้ยาที่มีฤทธิ์ต้านโคลิเนอร์จิก และความรู้ในการใช้ยาของผู้สูงอายุ เพื่อใช้เป็นแนวทางในการส่งเสริมความปลอดภัยและป้องกันอันตรายจากการใช้ยาในประชากรกลุ่มที่มีความเสี่ยงนี้


 การศึกษาเชิงบรรยายนี้ดำเนินการรวบรวมข้อมูลจากกลุ่มตัวอย่างผู้สูงอายุที่มีอายุตั้งแต่ 60 ปีขึ้นไป จำนวน 110 ราย โดยคัดเลือกแบบเฉพาะเจาะจงจากหน่วยตรวจผู้ป่วยนอกจำนวน 4 แผนก ณ โรงพยาบาลระดับตติยภูมิแห่งหนึ่งในกรุงเทพมหานคร ระหว่างเดือนพฤศจิกายน พ.ศ. 2563 ถึง 2565 เกณฑ์การคัดเข้าประกอบด้วย ผู้ที่ใช้ยากลุ่มต้านโคลิเนอร์จิกอย่างน้อย 1 รายการ สามารถสื่อสารด้วยภาษาไทยได้ดี มีความยินดีในการเข้าร่วมวิจัย และผ่านการคัดกรองสมรรถภาพทางสมองว่าอยู่ในเกณฑ์ปกติ โดยการใช้แบบประเมินสมรรถภาพการรู้คิด 6 ข้อฉบับภาษาไทย เพื่อยืนยันว่าผู้เข้าร่วมวิจัยไม่มีภาวะพร่องการรู้คิดที่อาจส่งผลต่อความเที่ยงตรงของข้อมูลที่ได้จากการสัมภาษณ์ สำหรับเกณฑ์การคัดออก คือ ผู้ที่ขอถอนตัวจากการเข้าร่วมวิจัย หรือปฏิเสธการให้ข้อมูลในระหว่างขั้นตอนการเก็บรวบรวมข้อมูล เครื่องมือวิจัยประกอบด้วยแบบสัมภาษณ์แบบมีโครงสร้างจำนวน 5 ชุด สำหรับเก็บรวบรวมข้อมูลส่วนบุคคล ข้อมูลการใช้ยาในปัจจุบัน การเกิดอาการไม่พึงประสงค์จากการใช้ยาต้านโคลิเนอร์จิก ร่วมกับการใช้เกณฑ์ของนารันโจ (Naranjo Scale) เพื่อการประเมินความน่าจะเป็นของการเกิดอาการไม่พึงประสงค์จากยา และข้อมูลความรู้ในการใช้ยา วิเคราะห์ข้อมูลด้วยสถิติบรรยาย ได้แก่ การแจกแจงความถี่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และพิสัย


ผลการศึกษาพบว่ากลุ่มตัวอย่างมีอายุเฉลี่ย 68.45 ปี ส่วนใหญ่เป็นเพศหญิง (ร้อยละ 68.18) และมีโรคประจำตัวร่วม 2-4 โรค โดยโรคความดันโลหิตสูงและไขมันในเลือดสูงเป็นภาวะที่พบมากที่สุดตามลำดับ สำหรับรายการยาต้านโคลิเนอร์จิกที่ใช้บ่อยที่สุดคือ lorazepam (ร้อยละ 45.45) โดยยากลุ่ม benzodiazepines มีสัดส่วนการใช้รวมสูงสุด (ร้อยละ 56.36) และพบอาการไม่พึงประสงค์จากการใช้ยาต้านโคลิเนอร์จิก ร้อยละ 12.73 ตามการรับรู้ของผู้สูงอายุ โดยอาการที่พบบ่อย ได้แก่ ตาพร่ามัว ปากแห้ง และเหนื่อยล้า ทั้งนี้ ผลการประเมินด้วยเกณฑ์นารันโจพบว่าอยู่ในระดับ “อาจจะใช่” (possible) และ “น่าสงสัย” (doubtful) เท่านั้น นอกจากนี้ยังพบว่ากลุ่มตัวอย่างเคยมีประสบการณ์อาการไม่พึงประสงค์ในอดีตจากการใช้ยากลุ่มนี้ตามการรับรู้ ร้อยละ 29.09 โดยอาการที่พบบ่อย 5 อันดับแรก คือ คลื่นไส้/อาเจียน ผื่น ตาพร่ามัว ปากแห้งและเหนื่อยล้า ในส่วนของระดับความรู้ แม้ผู้สูงอายุส่วนใหญ่ (เกินร้อยละ 80) มีความเข้าใจด้านสรรพคุณ วิธีใช้ และการเก็บรักษายา แต่มีเพียงร้อยละ 30 ที่ทราบชื่อยา และร้อยละ 73.64 ขาดความรู้เรื่องอาการไม่พึงประสงค์จากยา


ผลการศึกษาครั้งนี้ชี้ให้เห็นถึงความจำเป็นในการพัฒนาแนวทางการประเมินและติดตามการใช้ยาต้านโคลิเนอร์จิกอย่างใกล้ชิด รวมถึงการเฝ้าระวังอาการไม่พึงประสงค์อย่างเป็นระบบ ตลอดจนเป็นแนวทางสำหรับพยาบาลและบุคลากรอื่นๆ ที่ดูแลผู้สูงอายุในการส่งเสริมความรอบรู้ด้านยา เพื่อสร้างความปลอดภัยและลดความเสี่ยงจากการใช้ยาในประชากรผู้สูงอายุในระบบบริการสุขภาพไทย
