ผลลัพธ์ของผู้ปฏิบัติการพยาบาลขั้นสูงในการดูแลผู้ป่วยจิตเภท

Abstract

บทคัดย่อ

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลลัพธ์การปฏิบัติงานของผู้ปฏิบัติการ พยาบาลขั้นสูงในการดูแลผู้ป่วยจิตเภท กลุ่มตัวอย่างประกอบด้วย 4 กลุ่มคือ 1) ผู้ปฏิบัติการ พยาบาลขั้นสูง 1 ราย 2) เวชระเบียนของผู้ป่วยจิตเภทที่เข้ารับการรักษาในโรงพยาบาล 51 ราย 3) ผู้ป่วยจิตเภทและญาติผู้ดูแลที่ได้รับการพยาบาลจากผู้ปฏิบัติการพยาบาลขั้นสูง 69 ราย และ 4) ผู้มีส่วนเกี่ยวข้องกับผู้ปฏิบัติการพยาบาลขั้นสูง 14 ราย สถานที่วิจัยคือโรงพยาบาลเฉพาะทาง จิตเวชแห่งหนึ่งในภาคใต้ของประเทศไทย รวบรวมข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพ ข้อมูล เชิงปริมาณประกอบด้วย ผลลัพธ์ด้านผู้ป่วยและผู้ดูแล ได้แก่ ความสามารถในการดูแลตนเอง ของผู้ป่วยจิตเภท ภาระการดูแลของผู้ดูแลและความพึงพอใจในบริการที่ได้รับจากพยาบาล ส่วนผลลัพธ์ด้านองค์กร ได้แก่ การกลับมารักษาซ้ำ จำานวนวันนอน และค่าใช้จ่ายในการรักษา พยาบาล ข้อมูลเชิงคุณภาพรวบรวมโดยการสัมภาษณ์เชิงลึกและการสังเกตแบบไม่มีส่วนร่วม เพื่อรวบรวมข้อมูล โครงสร้างและกระบวนการบริการของผู้ปฏิบัติการพยาบาลขั้นสูงจากผู้มี ส่วนเกี่ยวข้องกับผู้ปฏิบัติการพยาบาลขั้นสูง วิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เนื้อหา ข้อมูล เชิงปริมาณ วิเคราะห์โดยสถิติเชิงพรรณนา สถิติ Two-way ANOVA และ Mann-Withney U test

ผลการศึกษาพบว่าผู้ปฏิบัติการพยาบาลขั้นสูงมีบทบาทสำคัญในการพัฒนาคุณภาพการ บริการขององค์กรให้มีมาตรฐานสอดคล้องกับมาตรฐานสากล ทาให้เกิดการยอมรับในคุณค่าของ ผู้ปฏิบัติการพยาบาลขั้นสูงจากองค์กรต่างๆ สามารถสร้างผลงานทางวิชาการให้กับทีมงานและ เผยแพร่สู่สาธารณะ ส่งผลให้องค์กรมีชื่อเสียง การประเมินผลลัพธ์ทั้งสองด้าน พบว่า หลังจากมี ผู้ปฏิบัติการพยาบาลขั้นสูงอยู่ในทีมของโรงพยาบาล ผู้ป่วยมีความสามารถในการดูแลตนเองระดับมาก ผู้ดูแลมีภาระการดูแลระดับปานกลาง และผู้ป่วยมีความพึงพอใจในบริการระดับสูง อัตราการกลับมา รักษาซ้ำ จำานวนวันนอนและค่าใช้จ่ายในการรักษาพยาบาลต่ำกว่าก่อนมีผู้ปฏิบัติการพยาบาลขั้นสูง ในทีมมีอย่างมีนัยสำาคัญทางสถิติ (p < .05, p < .001, และ p < .001)

ผลการศึกษาสนับสนุนประโยชน์ของการใช้ผู้ปฏิบัติการพยาบาลขั้นสูงสาขาการพยาบาล จิตเวชและสุขภาพจิตในการพัฒนาคุณภาพการดูแลผู้ป่วยจิตเภทในระบบสุขภาพ และขอเสนอ สภาการพยาบาลเร่งดำเนินการขอตำแหน่ง และศึกษาถึงการขยายขอบเขตของการพยาบาลขั้นสูง ในกลุ่มผู้ป่วยจิตเวชเพื่อการเข้าถึงบริการที่มีคุณภาพในระบบสุขภาพของประเทศไทย

Abstract

The purpose of this research was to investigate the outcomes of an advanced practice nurse (APN) in providing care for people with schizophrenia. The sample consisted of 1) one APN, 2) fifty one medical records of patients hospitalized with schizophrenia, 3) sixty nine patients with schizophrenia and their family caregivers, and 4) fourteen stakeholders who worked with the APN. The study was carried out in a psychiatric hospital in the south of Thailand. Both quantitative and qualitative data was collected. The quantitative data pertained to outcomes of patients and caregivers in their capacity of providing self care for the schizophrenic patients, the burden of the caregivers in providing care, and the satisfaction with nursing care provided by the nurse. The study assessed the organization by relapse rates, length of hospital stay, and cost of health care. Qualitative data pertaining to the structure and process of care provided by the APN was compiled through individual in-depth interviews and non-participatory observation. Qualitative data was analyzed by content analysis whereas quantitative data was analyzed using descriptive statistics and two-way ANOVA.

Results of the study revealed that the APN has an important role in the development of the quality of the services provided by the organization in meeting international standards. The value of the APN was recognized by various organizations, she was capable of making academic contributions to her team, and she contributed to the public recognition of her organization. In assessing the results of the study in both areas it was found that having an APN on the hospital team, the self-care ability and satisfaction were rated at high levels, the caregivers’ burden was at a moderate level, relapse rates, hospital stays, and hospital expenses were significant lower than prior to having an APN functioning on the team (p < .05, p < .001, and p < .001).

The findings support the APN’s contribution to the quality of care of patients with schizophrenia and mental health problems which resulted in developing a better quality health care system. It is recommended that the Thailand Nursing and Midwifery Council hasten to study and explore the need of expanding the scope of the APN in providing accessibility and quality care to patients with psychiatric problems as well as establishing APN positions within the health care system.
