บทคัดย่อ

          Neisseria gonorrhoeae เป็นหนึ่งในเชื้อก่อโรคติดต่อทางเพศสัมพันธ์ที่พบมากที่สุด การตรวจทางห้องปฏิบัติการที่รวดเร็วและมีประสิทธิภาพเป็นสิ่งสำคัญในการตรวจวินิจฉัย วิธีเพาะเชื้อยังคงเป็นวิธีมาตรฐานในการตรวจหาเชื้อ N. gonorrhoeae ขณะที่ในปัจจุบันวิธีการเพิ่มปริมาณสารพันธุกรรมเป็นวิธีที่มีความสำคัญมากขึ้น การศึกษาวิจัยนี้จึงมีวัตถุประสงค์เพื่อทดสอบสภาวะที่เหมาะสมและประเมินประสิทธิภาพร่วมกับการประเมินต้นทุนของชุดนํ้ายา in-house real time PCR ที่พัฒนาขึ้นเปรียบเทียบกับวิธีเพาะเชื้อ

          ตัวอย่างทดสอบในการศึกษานี้ ได้แก่ ตัวอย่างจากท่อปัสสาวะ ตัวอย่างจากปากมดลูก ตัวอย่างจากทวารหนัก และตัวอย่างจากช่องคอ จากผู้มารับบริการ ณ ศูนย์การแพทย์บางรักด้านโรคติดต่อทางเพศสัมพันธ์ ตัวอย่างทดสอบ 60 ตัวอย่างถูกนำมาใช้สำหรับการทดสอบสภาวะที่เหมาะสมของชุดนํ้ายา in-house real time PCR ขณะที่ 240 ตัวอย่างถูกนำมาใช้สำหรับการศึกษาเพื่อประเมินประสิทธิภาพของนํ้ายา ตัวอย่างทั้งหมด 300 ตัวอย่าง ถูกทดสอบด้วยนํ้ายา in-house real time PCR ที่พัฒนาขึ้นโดยมีการพัฒนาไพรเมอร์และโพรบซึ่งจำเพาะต่อยีน PorA สำหรับเชื้อ N. gonorrhoeae และจำเพาะต่อยีน human-beta globin สำหรับตัวควบคุมภายใน จากนั้นวิเคราะห์ค่าความไว ความจำเพาะ ค่าทำนายผลบวก และค่าทำนายผลลบเปรียบเทียบกับวิธีเพาะเชื้อ

         การศึกษานี้ใช้นํ้ายาสำเร็จรูป AnyplexTM CT/NG Real-time Detection ในกรณีที่ผลการทดสอบระหว่างสองวิธี ให้ผลไม่สอดคล้องกัน ผลการทดสอบพบว่า สภาวะความเข้มข้นปฏิกิริยาที่เหมาะสมสำหรับตรวจหาเชื้อ N.gonorrhoeae คือ ความเข้มข้น 200 nM และความเข้มข้น 200 nM สำหรับตัวควบคุมภายในเช่นเดียวกัน นํ้ายา in-house real time PCR ที่พัฒนาขึ้นสามารถตรวจหาเชื้อ N. gonorrhoeae และตัวควบคุมภายในได้ภายในหลอดปฏิกิริยาเดียวกัน ให้ค่าการประเมินประสิทธิภาพทุกค่าการทดสอบคิดเป็นร้อยละ 100 ในตัวอย่างส่งตรวจจากอวัยวะสืบพันธุ์ ขณะที่มีค่าความไว ความจำเพาะ ค่าทำนายผลบวก และค่าทำนายผลลบคิดเป็น 100, 86.67, 88.24, 100 ตามลำดับ สำหรับตัวอย่างจากทวารหนัก และร้อยละ 100, 96.67, 96.77, 100 ตามลำดับ สำหรับตัวอย่างจากช่องคอ การศึกษานี้พบผลที่ไม่สอดคล้องกันในตัวอย่างทดสอบ 5 ราย โดยเป็นตัวอย่างจากช่องทางอื่นนอกเหนือจากอวัยวะสืบพันธุ์ทั้งหมดซึ่งให้ผลไม่พบเชื้อ N. gonorrhoeae ด้วยวิธีเพาะเชื้อแต่พบเชื้อในการตรวจด้วยนํ้ายา in-house real time PCR และน้ำยาสำเร็จรูปที่ใช้หลักการเพิ่มปริมาณสารพันธุกรรม น้ำยา in-house real time PCR ที่พัฒนาขึ้น มีต้นทุน 165 บาทต่อการทดสอบ ขณะที่วิธีเพาะเชื้อมีต้นทุน 76 บาทต่อการทดสอบ และแม้ว่านํ้ายา in-house real time PCR ที่พัฒนาขึ้นจะมีราคาสูงกว่าวิธีเพาะเชื้อ แต่ในแง่ของการตรวจวินิจฉัยนํ้ายาที่พัฒนาขึ้นนี้มีความคุ้มทุนที่มากกว่าด้วยความไวและความจำเพาะในการตรวจพบเชื้อโดยเฉพาะในตัวอย่างช่องทางอื่นนอกเหนือจากอวัยวะสืบพันธุ์ 

          ข้อมูลจากการศึกษานี้แสดงให้เห็นว่าการตรวจหาเชื้อ N. gonorrhoeae ด้วยชุดนํ้ายา in-house real time PCR ที่พัฒนาขึ้นเป็นวิธีที่รวดเร็ว มีความไวและความจำเพาะในตัวอย่างทดสอบที่หลากหลายด้วยผลการทดสอบที่มีความถูกต้อง สอดคล้องกับวิธีมาตรฐาน ดังนั้นชุดนํ้ายา in-house real time PCR ที่พัฒนาขึ้นสำหรับตรวจหาเชื้อ N.gonorrhoeae สามารถนำมาใช้ในการตรวจวินิจฉัยในงานประจำวันได้
