การพยาบาลผู้ป่วยวัณโรคดื้อยารุนแรงหลายขนานชนิดรุนแรงมาก ในสถาบันบำราศนราดูร: กรณีศึกษา

บทความนี้มีวัตถุประสงค์เพื่อเป็นกรณีศึกษาการพยาบาลผู้ป่วยวัณโรคดื้อยารุนแรงหลายขนานชนิดรุนแรงมาก ในสถาบันบำราศนราดูร ซึ่งจะเป็นแนวทางในการพยาบาลผู้ป่วยวัณโรคดื้อยารุนแรงหลายขนานชนิดรุนแรงมากให้บุคลากรพยาบาล และเพื่อให้ผู้ป่วยและญาติมีความรู้ความเข้าใจในการปฏิบัติตัว โดยนำการประเมินแบบแผนสุขภาพของ Gordon มาใช้ในการประเมินอาการผู้ป่วย ใช้แบบประเมินภาวะโภชนาการ (Nutrition Alert Form, NAF) และแบบประเมินวางแผนบั้นปลายชีวิต จากการศึกษาผู้ป่วยรายนี้จึงได้การพยาบาลผู้ป่วยวัณโรคดื้อยาหลายขนานชนิดรุนแรงมาก ดังนี้ (1) การควบคุมการแพร่กระจายเชื้อวัณโรค (2) การสอบสวนโรคหรือการหาผู้สัมผัสร่วมบ้าน (3) การกำกับการรับประทานยาอย่างเข้มข้นและมีคุณภาพ (4) การติดตามประเมินผลข้างเคียงของยาที่ผู้ป่วยได้รับ (5) การดูแลภาวะโภชนาการ (6) การดูแลแบบประคับประคอง และ (7) การช่วยเหลือด้านเศรษฐกิจและสังคม การรักษาวัณโรคดื้อยารุนแรงหลายขนานชนิดรุนแรงมาก นอกจากที่จะต้องใช้เวลาในการรักษาอย่างยาวนานและอาศัยความร่วมมือในการดูแลของทีมสหสาขาวิชาชีพแล้ว ควรดำเนินการให้ผู้ป่วยมีวินัยในการรับประทานยาให้ได้ต่อเนื่องสม่ำเสมอตามแนวทางการรักษา
