Abstract

          The objectives of this research were to develop a care model for people living with HIV and AIDS and apply the developed care model for people living with HIV and AIDS. The sample group comprised of the quality improvement team for 12 clinical, patient and 89 ART recipients. Research tools included mind map, group discussion, information gathering from operational documents,guidelines, observations, and activity logs. Qualitative data was analyzed by classification inductive analysis, interpreting, constructing conclusions and explaining phenomena quantitative data used frequency analysis percentage and mean. The research was conducted during October 2020 to April 2021.

          The results showed that the clinical care quality improvement team developed the care model for HIV and AIDS patients, including a contextual study, a practice model, analyzing service access problems, continuum care service. Research procedures provided increasing knowledge, raising awareness of the team to improve clinical patient care quality and build a wider network in the community. Made an order to appoint a committee to improve the quality of care for HIV and AIDS patients. Training clinical patients care for HIV coordinator (HIV Co) and the care team, updating and promulgating Clinical Practice Guideline (CPG).
After applying the developed model, there were 391 cases voluntarily tested for HIV and two cases with
AIDS-related symptoms disclosed their HIV infection status to their families and willing to be tested at the hospital and found out that both of them were positive. They were treated by antiretroviral therapy. The success rate of HIV infected person taking antiretroviral drugs accounted for 93.24 percent.
