Malnutrition in Pre-dialysis Chronic Kidney Disease Patients at National Institute for Nephrology and Transplantation, Sri Lanka: Prevalence and Its Associated Factors

บทคัดย่อ

Background: Malnutrition is a recognized complication of chronic kidney disease (CKD). This study  was carried out to determine the prevalence of malnutrition and its associated factors among pre-dialysis  CKD patients.  

Methods: A cross-sectional study was carried out on pre-dialysis CKD patients at the National Institute  for Nephrology, Dialysis and Transplantation (NINDT), Sri Lanka. Systematic sampling method was used.  Patients’ clinical, anthropometric, laboratory parameters were assessed. Nutritional status was evaluated  using ‘Subjective Global Assessment’ tool.  

Results: The response rate was 98%. The mean age of the CKD patients was 57.4 ± 12.6 years with  a male: female ratio of 1.6:1. Diabetes mellitus was the most common co-morbidity associated with CKD.  (n = 180, 90.5%) Only 7.5% (n = 15) engaged in moderate intensity exercise. The mean serum albumin level  was 3.7 g/dl (SD = 0.7). Hypoalbuminemia detected in 40.7% (n = 74) patients. Prevalence of malnutrition  was 62% (95%CI:54.9 - 68.7). The prevalence of malnutrition increased significantly across CKD stages  (p = 0.001). It was significantly commoner in patients without a family history of CKD (p = 0.011) and who  referred to nutritional counselling (p = 0.017). More severe the CKD stage (p = 0.041), referred to nutrition  counselling (p = 0.017) and absence of a family history of CKD (p = 0.021) were significantly associated  with higher risk of malnutrition independent of other covariates.  

Conclusion: The prevalence of malnutrition among pre-dialysis CKD patients was high. There was a  significant association between the prevalence of malnutrition and worsening kidney function, family history  of CKD and referred to nutrition counselling.
