Care Model for Patients with Chronic Obstructive Pulmonary Disease In ThaWangPha Hospital and in Community

Abstract

This article analyzed six research reports related to development of care model for patients with COPD in Tha Wang Pha hospital and in community during 9 years of development (2004-2013).  The lessons’ learned from the development of the care model based on researchers’ experiences and suggestions were reported in order to further develop the care model sustainably.
