Success Roles of APN in Using Case Management

Abstract

 Aims of health care service in the present focus on quality, effectiveness, accessibility and cost benefit. Case management is a system that can pave the way to these aims. It is an important of APNs role, and a role of success in multidisciplinary team work. Since this system needs closely and effective team leading by case manager and using Care of Multidisciplinary Approach Planning / Care MAP.  So, Success in this role Advanced Practice Nurse-Case Manager /APN-CM needs to practice very well not only direct and indirect patient care, but also management in coordination, information, communication, change and financial.
