Effect of Behavior Modification Program Led By Advanced Practice Nurse on Clinical Outcomes in Patients with Pre-dialysis Chronic Kidney Disease.

Abstract

This quasi experimental research aimed to examine the effect of behavior modification program which integrated Orem’s theory of self-care, motivational interviewing and roles of advanced practice nurse, on outcomes of patients with pre-dialysis chronic kidney disease. The samples were 75 patients with chronic kidney disease (CKD) and were recruited based on selective criteria from kidney disease clinic of a tertiary care hospital in Bangkok during September 2016 to June 2017. The program comprised of 5 activities in 6 months including: 1) providing CKD knowledge 2) self-awareness 3) building motivation 4) self-care teaching and 5) continuing self-care support. Research instruments included: demographic and health history form, CKD knowledge assessment, readiness to behavior change form, self-care behavior form, and records of patients’ outcomes and advices. Results showed that CKD knowledge of the sample were increased at immediate after the teaching and at 6th month period. Self-care behaviors were also increased significantly at 3rd month and at 6th month (p =.000). Percentages of patients with glomerular filtration rate, blood pressure, and HbA1c within the standard criteria were also increased. The behavior modification program led by advanced practice could enhance patients’ motivation, self-care abilities, and tend to slow the progression of CKD.
