ที่มา: แนวปฏิบัติการพยาบาลผู้ป่วยผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ ที่ใช้ในโรงพยาบาลสุราษฎร์ธานี ไม่ครอบคลุมการดูแลในทุกระยะของการผ่าตัด ส่งผลให้ผู้ป่วยมีภาวะแทรกซ้อนที่ต้องเข้ารักษาซ้ำในโรงพยาบาล
วิธีการศึกษา: การวิจัยครั้งนี้เป็นการวิจัยและพัฒนา เพื่อพัฒนาแนวปฏิบัติการพยาบาลผู้ป่วยผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ ในโรงพยาบาลสุราษฎร์ธานี ที่ครอบคลุมระยะก่อนผ่าตัด ระยะเข้ารับการผ่าตัด และระยะหลังจำหน่ายจากโรงพยาบาล และประเมินผลการใช้แนวปฏิบัติการพยาบาล กลุ่มตัวอย่างมี 2 กลุ่ม คือ 1) ผู้ป่วยโรคที่เข้ารับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ จำนวน 32 คน 2) พยาบาลวิชาชีพที่ให้การดูแลผู้ป่วยก่อนและหลังผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ หัวใจ จำนวน 52 คน ทำการเก็บข้อมูลระหว่าง เดือน สิงหาคม 2567-มกราคม 2568 เครื่องมือที่ใช้ในการวิจัยประกอบด้วย 1) แบบบันทึกข้อมูลส่วนบุคคล 2) แนวปฏิบัติการพยาบาลผู้ป่วยผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ 3) แบบประเมินความพึงพอใจในการใช้แนวปฏิบัติการพยาบาล และ 4) แบบประเมินความรู้การพยาบาลผู้ป่วยผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ การวิเคราะห์ข้อมูลใช้สถิติเชิงพรรณนา สถิติการทดสอบค่าทีแบบอิสระ (Independent t-test) และสถิติไคสแควร์ (Chi-square)
ผลการวิจัย: ผู้ป่วยที่เข้ารับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ ร้อยละ 100 ได้รับการผ่าตัดตามกำหนด ไม่พบกรณีการเลื่อนผ่าตัดเนื่องจากการเตรียมตัวไม่พร้อม วันนอนเฉลี่ยในหอผู้ป่วยวิกฤตหลังผ่าตัด วันนอนเฉลี่ยในโรงพยาบาลหลังผ่าตัด และจำนวนวันใส่ท่อช่วยหายใจเฉลี่ย ก่อนการใช้แนวปฏิบัติการพยาบาลและหลังการพัฒนามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p<.05) นอกจากนี้ จำนวนครั้งของการเกิดภาวะแทรกซ้อนหลังการจำหน่ายจากโรงพยาบาลภายหลังการใช้แนวปฏิบัติ แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p<.05) สำหรับพยาบาลวิชาชีพ พบว่า คะแนนความรู้เกี่ยวกับการพยาบาลผู้ป่วยผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจก่อนและหลังการใช้แนวปฏิบัติการพยาบาล มีความแตกต่างอย่างมีนัยสำคัญทางสถิติ (p<.01) และพยาบาลวิชาชีพมีความพึงพอใจในการใช้แนวปฏิบัติการพยาบาลในระดับมากที่สุด ( = 4.73, S.D. = 0.27)
สรุป: แนวปฏิบัติการพยาบาลผู้ป่วยผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจที่ได้รับการพัฒนา สามารถใช้เป็นแนวทางในการปฏิบัติการพยาบาลที่ครอบคลุมระยะก่อนผ่าตัด ระยะเข้ารับการผ่าตัด และระยะหลังจำหน่ายจากโรงพยาบาลได้
