บทคัดย่อ

การดูแลผู้สูงอายุติดเตียง เป็นการดูแลที่ยุ่งยากและใช้ระยะเวลาที่ยาวนาน ส่งผลกระทบให้ผู้ดูแลหลักเกิดปัญหาทางสุขภาพกายและสุขภาพจิต จนเกิดเป็นภาระการดูแล การวิจัยเชิงทำนายครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาภาระการดูแลในผู้ดูแลผู้สูงอายุติดเตียง และศึกษาอำนาจการทำนายภาระการดูแลในผู้ดูแลผู้สูงอายุติดเตียงของการสนับสนุนทางสังคม ความเครียด และระยะเวลาการดูแล กลุ่มตัวอย่าง คือ ผู้ดูแลผู้สูงอายุติดเตียงที่เป็นสมาชิกครอบครัว ปฏิบัติหน้าที่หลักในการดูแลมาแล้วอย่างน้อย 3 เดือน ในจังหวัดนครสรรค์ โดยไม่ได้รับค่าตอบแทนในการดูแล จำนวน 120 คน ผู้วิจัยคัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง เครื่องมือที่ใช้ในการรวบรวมข้อมูล ประกอบด้วย แบบรวบรวมข้อมูลส่วนบุคคล แบบประเมินภาระการดูแล (ZBI) แบบประเมินการสนับสนุนทางสังคม (MSPSS) และแบบประเมินความเครียด (PSS) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติ multiple linear regression analysis

ผลการวิจัยพบว่า ผู้ดูแลผู้สูงอายุติดเตียงมีคะแนนเฉลี่ยภาระการดูแลอยู่ในระดับต่ำถึงปานกลาง (M = 24.89, SD = 15.27) การสนับสนุนทางสังคม ความเครียด และระยะเวลาในการดูแล สามารถร่วมกันทำนายภาระการดูแลได้ร้อยละ 29 โดยความเครียด สามารถทำนายภาระการดูแลได้มากที่สุด (β = 0.51, p < 0.01) รองลงมา คือ การสนับสนุนทางสังคมสามารถทำนายภาระการดูแลได้ (β = -0.17, p = 0.03) ในขณะที่ระยะเวลาการดูแลในหนึ่งวันไม่สามารถทำนายภาระการดูแลได้ (β = -0.05, p = 0.54)

ผลการศึกษาครั้งนี้ สามารถเป็นข้อมูลพื้นฐานสำหรับพยาบาล บุคลากรทางการแพทย์ และผู้ออกแบบนโยบายสาธารณสุข เพื่อออกแบบและพัฒนาการให้บริการสุขภาพ ส่งเสริมให้ผู้ดูแลผู้สูงอายุติดเตียงเข้าถึงการสนับสนุนทางสังคม และสามารถจัดการกับความเครียด เพื่อลดภาระการดูแล ทำให้ผู้สูงอายุกลุ่มติดเตียงได้รับการดูแลที่มีประสิทธิภาพต่อไป
