บทคัดย่อ
บทนำ
การซ่อมเสริมภาวะวิการหลังผ่าตัดมะเร็งศีรษะและคอมีความซับซ้อนและท้าทายเป็นอย่างมาก นอกจากแพทย์ต้องเย็บปิดเนื้อเยื่อภายในทางเดินอาหารไม่ให้เกิดการรั่วซึมของน้ำลายแล้ว ยังต้องใส่ใจรายละเอียดในวิธีการซ่อมเสริมเพื่อคงการทำงานอวัยวะที่เกี่ยวข้องกับการกลืน การหายใจ และการพูด รวมถึงคงความสวยงามของใบหน้าให้ใกล้เคียงปกติ
Free flap ถือเป็นมาตรฐานในการผ่าตัดซ่อมเสริมบริเวณศีรษะและคอ เนื่องจากให้ผลลัพธ์ที่ดีกว่าการซ่อมเสริมด้วยวิธีอื่นมาก แต่การผ่าตัด Free flap มีข้อจำกัดหลายประการเช่น อาศัยความชำนาญในการเย็บหลอดเลือดขนาดเล็ก (microvascular surgery) ใช้เวลาผ่าตัดยาวนานกว่า ต้องมีการดูแลหลังผ่าตัดอย่างใกล้ชิด บ่อยครั้งผู้ป่วยเป็นมะเร็งเป็นซ้ำหรือเคยผ่าตัดรักษามาก่อนหรือมีโรคประจำตัวที่ไม่สามารถทนการผ่าตัดที่ยาวนานได้ การผ่าตัดซ่อมเสริมด้วย free flap จึงไม่สามารถทำได้ในผู้ป่วยทุกราย ในปัจจุบันมี regional flap ที่น่าสนใจเช่น supraclavicular flap ซึ่งเนื้อเยื่อมีความบางและผ่าตัดได้ง่าย มีขั้วเส้นเลือดที่ยาวสามารถใช้ซ่อมภาวะวิการบริเวณศีรษะและคอได้หลายตำแหน่ง รวมถึงสามารถเย็บปิด donor site ได้(1, 2) ในบทความนี้จะบรรยายถึงลักษณะกายวิภาค วิธีการผ่าตัด ข้อบ่งชี้และตัวอย่างผู้ป่วยที่ซ่อมเสริมด้วย supraclavicular flap
