การวิจัยเชิงปฏิบัติ: การใช้กรอบ CFIR เพื่อพัฒนาคุณภาพการพยาบาลในบริบทการปฏิบัติจริง

บทคัดย่อ
ความรู้ทางการพยาบาลได้มีการพัฒนาอย่างต่อเนื่องในช่วงทศวรรษที่ผ่านมา โดยเฉพาะอย่างยิ่งเมื่อมีการนำความรู้จากงานวิจัยและหลักฐานเชิงประจักษ์มาเป็นองค์ประกอบหลักในการพัฒนาคุณภาพทางการพยาบาลในทุกกลุ่มประชากร อย่างไรก็ตาม ช่องว่างระหว่างความรู้ที่ได้จากการวิจัยกับการนำไปปฏิบัติจริง (Knowledge-to-practice gap) ระหว่างงานวิจัยและหลักฐานเชิงประจักษ์กับสถานการณ์จริงในการปฏิบัติงานในอีกหลายพื้นที่ยังคงเป็นอุปสรรคสำคัญอย่างต่อเนื่อง ส่งผลการเปลี่ยนแปลงระบบบริการสุขภาพเกิดความไม่ยั่งยืน ปัจจุบันจึงมีความพยายามในการศึกษาในเรื่อง “งานวิจัยเพื่อการนำไปใช้ หรือ Implementation Research (IR) เพื่อการประเมินความพร้อม ติดตามประเมินปัญหาและอุปสรรค ของการพัฒนาคุณภาพกรณีนำงานวิจัย หรือ หลักฐานเชิงประจักษ์ไปใช้ในสถานการณ์จริง การศึกษานี้มีวัตถุประสงค์เพื่อนำเสนอการทบทวนวรรณกรรมและงานวิจัยเกี่ยวกับกระบวนการและปัจจัยที่มีผลต่อการนำความรู้เชิงประจักษ์ไปใช้ในทางการพยาบาล โดยอิงตามกรอบการวิจัยการนำไปปฏิบัติแบบบูรณาการ (Consolidated Framework for Implementation Research; CFIR) โดยใช้วิธีการทบทวนวรรณกรรมและงานวิจัยที่เกี่ยวข้องกับการประยุกต์ใช้กรอบ CFIR เป็นแนวทางในการพัฒนาคุณภาพทางการพยาบาล ผลการศึกษา พบว่า การใช้กรอบ CFIR เป็นกลไกสำคัญที่ช่วยลด “ช่องว่างระหว่างความรู้และการปฏิบัติ” ในการปฏิบัติจริง รวมถึงพัฒนาและประเมินกลยุทธ์เพื่อให้การนำหลักฐานเชิงประจักษ์ไปใช้มีประสิทธิภาพและยั่งยืน ท้ายสุดบทความได้ให้ข้อเสนอแนะเชิงนโยบายในการส่งเสริมความสำเร็จและความยั่งยืนของการพัฒนาคุณภาพทางการพยาบาลโดยใช้หลักฐานเชิงประจักษ์เพื่อเป็นแนวทางสำหรับพยาบาลในการพัฒนาคุณภาพทางการพยาบาลอย่างยั่งยืน
