บทบาทของชุมชนในการดูแลหญิงตั้งครรภ์ที่ใช้สารเสพติดโดยการมีส่วนร่วมของชุมชน

การใช้สารเสพติดในหญิงตั้งครรภ์เป็นปัญหาสาธารณสุขที่มีความซับซ้อนและส่งผลกระทบต่อเนื่องทั้งในมิติสุขภาพและมิติทางสังคมเศรษฐกิจ แม้ว่าทฤษฎีการมีส่วนร่วมของชุมชนจะถูกนำมาใช้เป็นกลไกหลักในการแก้ไขปัญหา แต่หลักฐานเชิงประจักษ์ยังมีความไม่สอดคล้องและมีช่องว่าง เนื่องจากรูปแบบการดำเนินงานส่วนใหญ่ยังติดกับดักการมีส่วนร่วมเชิงสัญลักษณ์ ขาดการเสริมสร้างพลังอำนาจตัดสินใจ บทความนี้มีวัตถุประสงค์เพื่อวิเคราะห์บทบาทของชุมชนในการดูแลหญิงตั้งครรภ์ที่ใช้สารเสพติดและระบุปัจจัยความท้าทายภายใต้กรอบทฤษฎีการมีส่วนร่วมของชุมชน โดยการทบทวนบทความทางวิชาการ รายงานการวิจัย และกรณีศึกษาที่เกี่ยวข้อง ผลการศึกษาพบว่า บทบาทชุมชนในการดูแลหญิงตั้งครรภ์ที่ใช้สารเสพติดปรากฏใน 5 มิติหลัก ได้แก่ การวางแผน การดำเนินงาน การใช้ประโยชน์ การรับผลประโยชน์ และการประเมินผล ผ่านการขับเคลื่อนกลไกโมเดล 4M ทั้งในด้านการส่งเสริมความรอบรู้ด้านสุขภาพ การเฝ้าระวัง การประสานความร่วมมือระหว่างภาคส่วน และการสร้างแรงจูงใจพร้อมการสนับสนุน อย่างไรก็ตาม ข้อค้นพบเชิงวิเคราะห์สำคัญ ชี้ให้เห็นว่าการขับเคลื่อนในพื้นที่ยังเผชิญข้อจำกัดเชิงโครงสร้าง ทั้งความเหลื่อมล้ำเชิงอำนาจ ข้อจำกัดด้านการรักษาความลับ และความเสี่ยงต่อการเกิดตราบาปซ้ำ จึงเสนอแนะการบูรณาการทุนทางสังคม ร่วมกับการปรับปรุงมาตรการลดการตีตรา สร้างระบบการดูแลที่ไร้รอยต่อและยั่งยืนตามบริบททางวัฒนธรรม เพื่อยกระดับจากการมีส่วนร่วมเชิงสัญลักษณ์ไปสู่การเสริมพลังอำนาจ อันจะนำไปสู่ระบบบริการสุขภาพที่ยกระดับคุณภาพชีวิตของมารดาและทารกอย่างเป็นรูปธรรม
