Role of Physical Therapist Service for Elders Care and Working with Multidisciplinary Care Team in Geriatric Clinic at King Taksin Memorial Hospital in Complete Aged Society

Thailand has officially entered a complete aged society. At King Taksin Memorial Hospital, physical therapists at the Geriatric Clinic initially provided reactive care based on referrals-focusing on assessment, basic treatment, and advice. However, this limited approach was not sufficient for elderly patients with complex health issues requiring holistic and multidisciplinary care to maintain quality of life. To solve this, physical therapists developed a proactive care model covering two key areas: falling risk screening and physical therapy assessment. Those who were at risk of falling receive feedback, advice and balance training. Patients with physical therapy-related problems are given self-care advice, basic treatment, and referred when necessary. Follow-ups are scheduled, and case conferences are held with the multidisciplinary team, including home visits. A guideline for physical therapy services in collaboration with the Geriatric Clinic was established to support consistent, effective collaboration within the clinic. This ensures elderly patients receive appropriate and safe care with professional standards. Additionally, a book and e-book “Know Before You Grow... Old” were produced to educate the elderly with pictures and practical instructions. This article aims to present a framework for elderly care derived from the knowledge and experience of physical therapists who have worked at the Geriatric Clinic of King Taksin Memorial Hospital over the past ten years. The objective is to offer a guideline for other physical therapists, as well as medical personnel interested in applying these insights to improve elderly care services. Additionally, the article introduces an electronic book as a supporting material, which can be utilized or disseminated to those interested in the subject.
